雷氏隔药隔姜灸脐疗法对缓解期溃疡性结肠炎患者的疗效研究

摘　　要：目的观察雷氏隔药隔姜灸脐疗法治疗缓解期溃疡性结肠炎(Ulcerative colitis,UC)患者的临床疗效。方法选择2018年1月-2021年1月唐山市丰润区第二人民医院消化内科收治的90例缓解期UC患者为研究对象,按照随机数字表法分为2组,灸脐疗法组45例采用雷氏隔药隔姜灸脐疗法联合美沙拉嗪肠溶片治疗,对照组45例采用美沙拉嗪肠溶片治疗,共治疗4周记录12个月内的复发率、初次复发时间及复发时疾病活动程度;测定治疗前后2组血管内皮生长因子(VEGF)、黏膜地址素细胞黏附分子-1(Madcam-1)、血清肿瘤坏死因子-α(TNF-α)、白细胞介素-8(IL-8)含量变化情况及外周血T细胞17(Th17)和调节性T细胞(Treg)的表达情况。结果与治疗前比较,两组患者治疗后IL-8、TNF-α、VEGF、Madcam-1、Th17细胞水平均降低,Treg细胞水平升高,差异有统计学意义(P<0.05)。与对照组治疗后比较,灸脐疗法组治疗后IL-8、TNF-α、VEGF、Madcam-1、Th17细胞水平均降低,Treg细胞水平升高,差异有统计学意义(P<0.05)。与对照组比较,灸脐疗法组复发率降低,平均复发次数减少,初次复发时间缩短,患者在复发时病情活动情况减轻,差异有统计学意义(P<0.01)。结论雷氏隔药隔姜灸脐疗法能通过抑制炎症因子,降低血管损伤,改善免疫状态,进一步降低复发率、减少复发次数,推迟复发时间,减缓复发时的病情活动度,维持UC患者良好的缓解状态。ObjectiveTo observe the clinical effect of LEI′s medicine and ginger-umbilical moxibustion therapy on patients with ulcerative colitis(UC)in remission.Methods90 patients with remission UC admitted to the Department of Gastroenterology of the Second People′s Hospital of Fengrun District,Tangshan City from January 2018 to January 2021 were selected as the study subjects.They were divided into two groups according to the method of random number table.45 patients in the navel moxibustion therapy group were treated with Rayleigh′s medicine-separated ginger moxibustion therapy combined with mesalazine enteric-coated tablets,while 45 patients in the control group were treated with mesalazine enteric-coated tablets.The recurrence rate within 12 months was recorded for 4 weeks The time of initial recurrence and the degree of disease activity at the time of recurrence;Determination of vascular endothelial growth factor(VEGF),mucosal address cell adhesion molecule-1(Madcam-1)and serum tumor necrosis factor(TNF)before and after treatment in two groups-α(TNF-α).The change of interleukin-8(IL-8)content and the expression of peripheral blood T cell 17(Th17)and regulatory T cell(Treg).ResultsCompared with before treatment,IL-8 and TNF of patients in both groups after treatment-α.The levels of VEGF,Madcam-1 and Th17 cells were decreased,while the level of Treg cells was increased,the difference was statistically significant(P<0.05).Compared with the control group after treatment,IL-8 and TNF in the moxibustion therapy group after treatment-α.The levels of VEGF,Madcam-1 and Th17 cells were decreased,while the level of Treg cells was increased,the difference was statistically significant(P<0.05).Compared with the control group,the recurrence rate,the average number of recurrence,the time of initial recurrence and the activity of patients at the time of recurrence in the moxibustion navel therapy group were significantly lower(P<0.01).ConclusionLEI′s medicine and ginger-umbilical moxibustion therapy can further reduce th
