摘　　要：目的研究趋化因子C‑X‑C基元受体6(chemokine C‑X‑C motif receptor 6,CXCR6)的表达与非小细胞肺癌(non‑small cell lung cancer,NSCLC)临床病理特征的关系,并探讨CXCR6表达、CD4^(+)T细胞和CD8^(+)T细胞浸润在NSCLC转移中的作用,为NSCLC发生、转移和预后判断提供依据。方法采用免疫组织化学法检测122例NSCLC原发灶、47例相应淋巴结转移灶及21例癌旁正常组织中CXCR6的表达情况及癌性间质中CD4^(+)T细胞、CD8^(+)T细胞的浸润情况。结果癌细胞中CXCR6的表达水平在Ⅲ~Ⅳ期、有淋巴结转移组高于Ⅰ~Ⅱ期、无淋巴结转移组;癌细胞中CXCR6的表达水平高于癌性间质及癌旁正常肺组织;原发灶CXCR6的表达水平高于淋巴结转移灶;癌组织中浸润的T细胞中CD4、CD8表达水平均高于癌旁正常肺组织;CD4与癌细胞及癌性间质中CXCR6表达水平存在相关性。结论在NSCLC癌细胞中CXCR6高表达者容易发生淋巴结转移,CXCR6在转移中起着重要作用;癌细胞与癌性间质中CXCR6的表达在肿瘤免疫方面可能起着相反的作用。Objective:To study the relationship between chemokine C‑X‑C motif receptor 6(CXCR6)expression and clinicopathological features of non-small cell lung cancer,and to explore the role of CXCR6 expression,CD4^(+)T cells and CD8^(+)T cells infiltration in metastasis of NSCLC,so as to provide basis for the occurrence,metastasis and prognosis of NSCLC.Methods:Immunohistochemistry was used to detect the expression of CXCR6 in 122 cases of primary NSCLC,47 cases of corresponding lymph node metastasis and 21 cases of normal tissues adjacent to cancer,and the infiltration of CD4^(+)T cells and CD8^(+)T cells in cancerous stroma.Results:The expression level of CXCR6 in cancer cells in stageⅢ~Ⅳwith lymph node metastasis was higher than that in stageⅠ~Ⅱwithout lymph node metastasis.The expression level of CXCR6 in cancer cells was higher than that in cancer stroma and adjacent normal lung tissues.The expression level of CXCR6 in primary lesion was higher than that in lymph node metastasis lesion.The expression levels of CD4 and CD8 of infiltrated T cells in cancer tissues were higher than those in adjacent normal lung tissues.There is a correlation between CD4 and CXCR6 expression in cancerous stroma and cancer cells.Conclusion:The high expression of CXCR6 in NSCLC is prone to lymph node metastasi and plays an important role in metastasis.The expression of CXCR6 in cancer cells and cancerous stroma may play opposite roles in tumor immunity.
