《金匮要略》“历节”病名新解  

New explanation of the disease name of‘multiple arthralgia’in Synopsis of Golden Chamber

摘　　要：《金匮要略》中有“历节”之病名,对于“历节”之“历”,后世医家多训为“经历”或“遍历”,即风邪经过关节作痛或遍身关节作痛之意。文章通过文字学考证,结合张仲景所描述“历节”之疾病特点,认为“历节”之“历”或为“鬲”之通假,“历节”实为“鬲节”,相当于“如鬲(足)之节”或“其节如鬲(足)”。其足肥大是鬲形制上最大的特征,“历节”之名当指患者之关节如鬲足般肿大之意。这一病名形象地突出了历节病关节肿胀的临床特点。同时,鬲足肥大而中空特征,也暗合了对历节患者形盛而体虚的隐喻。这一解释或更加贴近张仲景对历节命名的原意。Synopsis of Golden Chamber has the disease name of‘multiple arthralgia’.For the‘Li’of‘Li Jie’,later physicians often explain it as‘pass by’or‘spread all over’,which mean pain caused of wind evil passed by the joints or the joints pain all over.Through textual research,combined with the disease characteristics of‘multiple arthralgia’described by ZHANG Zhong-jing,this paper holds that the‘Li’of‘multiple arthralgia’is an interchangeable words of‘Li pot(a kind of ancient cooking tripod with hollow legs)’.The large foot is the biggest feature of the Li pot’s shape,‘multiple arthralgia’is actually means the patient’s joints swelling as Li pot’s foot.This disease name vividly highlights the clinical characteristics of joint swelling in multiple arthralgia.At the same time,the characteristics of large and hollow foot also coincides with the metaphor of the multiple arthralgia patients are outwardly strong but inwardly weak.This explanation may be closer to ZHANG Zhong-jing’s original meaning of naming multiple arthralgia.
