摘 要:目的探讨红芪干预心肌梗死后心力衰竭的可能作用机制。方法120只大鼠随机分为造模组110只和假手术组10只。造模组采用结扎大鼠左冠状动脉前降支建立心梗后心力衰竭模型。75只大鼠建模成功后随机分为红芪组、半剂量红芪组、黄芪组、曲美他嗪组、模型组,每组15只。红芪组给予红芪提取液1.5 g/(kg·d)灌胃;半剂量红芪组给予红芪提取液0.75 g/(kg·d)灌胃;黄芪组给予黄芪提取液1.5 g/(kg·d)灌胃;曲美他嗪组给予曲美他嗪7.5 mg/(kg·d)灌胃;模型组和假手术组给予10 ml/(kg·d)生理盐水灌胃。灌胃2周后测量各组大鼠心功能、血清游离乳酸(LAC)、血清游离脂肪酸(NEFA)含量。结果与假手术组比较,模型组左室射血分数(EF)、短轴缩窄率(FS)明显降低,左室收缩末期内径(LVIDs)、左室舒张末期内径(LVIDd)、脂肪酸(FFA)明显升高(P<0.05)。与模型组比较,红芪组EF、FS、LAC明显升高,LVIDs、FFA明显降低;曲美他嗪组EF和FS明显升高,LVIDs、LVIDd、FFA明显降低;黄芪组FFA明显升高(P<0.05或P<0.01)。与半剂量红芪组比较,红芪组EF和FS明显升高,FFA明显降低(P<0.05)。各组大鼠左室收缩末期后壁厚度(LVPWs)、左室舒张末期后壁厚度(LVPWd)差异无统计学意义(P>0.05)。与黄芪组比较,红芪组FFA显明显降低(P<0.01)。结论红芪可提升心肌梗死后心力衰竭的心脏射血分数,改善心脏能量代谢重构,其作用机制可能与增加脂肪酸氧化供能、降低乳酸有关。Objective To investigate the possible mechanism of Hongqi(Radix Hedysari) in treatment of heart failure after myocardial infarction. Methods A total of 120 rats were randomly divided into the modeling group(n=110) and sham operation group(n=10). The rat model of heart failure after myocardial infarction was established by ligating the left anterior descending coronary artery. Seventy-five successfully modeled rats were randomly divided into Hongqi group, Hongqi half-dose group, Huangqi(Radix Astragali) group, trimetazidine group and model group, with 15 rats in each group. Hongqi group was given 1. 5 g/(kg·d) of Hongqi extract by gavage, while the half-dose group was given 0. 75 g/(kg·d) by gavage;Huangqi group was given 1. 5 g/(kg·d) of Huangqi extract by gavage;trimetazidine group was given 7. 5 mg/(kg·d) of trimetazidine by gavage;rats in the model group and sham operation group were given 10 ml/(kg·d) of normal saline by gavage. After two weeks, the cardiac function, serum lactic acid(LAC) and free fatty acids(FFA) were measured. Results Compared to those in the sham operation group, the left ventricular ejection fraction(EF) and left ventricular shortening rate(FS) in the model group decreased significantly, while the left ventricular end systolic diameter(LVIDs), left ventricular end diastolic diameter(LVIDd) and FFA increased significantly(P<0. 05). Compared to those in the model group, the EF, FS and LAC in the Hongqi group increased significantly, while the LVIDs and FFA decreased significantly;the EF and FS in the trimetazidine group increased significantly, while the LVIDs, LVIDd and FFA were reduced;the FFA in the Huangqi group increased significantly(P<0. 05 or P<0. 01). Compared to those in the half-dose Hongqi group, the EF and FS in the Hongqi group increased significantly, while the FFA decreased(P<0. 05). There was no significant difference in left ventricular posterior wall end systole(LVPWs) and left ventricular posterior wall end diastole(LVPWd) among the rats in each group(P>0. 05). Com
