摘　　要：目的分析催乳素细胞腺瘤患者胰岛素样生长因子-1(insulin-like growth factor 1,IGF-1)和胰岛素样生长因子结合蛋白(insulin-like growth factor binding protein 3,IGFBP-3)的表达及其临床意义。方法选取2018年1月至2021年12月昆明医科大学第二附属医院收治的60例催乳素细胞腺瘤患者作为研究组,选取同期进行健康体检的60名志愿者作为对照组。比较研究组及对照组IGF-1和IGFBP-3的表达情况及IGF-1/IGFBP-3比值。结果研究组IGF-1表达水平及IGF-1/IGFBP-3比值均高于对照组,差异均有统计学意义(P<0.05),但两组IGFBP-3表达水平比较,差异无统计学意义(P>0.05);不同性别的催乳素细胞腺瘤患者IGF-1表达水平及IGF-1/IGFBP-3比值比较,差异无统计学意义(P>0.05),侵袭性肿瘤患者的IGF-1表达水平及IGF-1/IGFBP-3比值较非侵袭性肿瘤患者高,差异均有统计学意义(P<0.05)。通过受试者操作特征(receiver operating characteristic,ROC)曲线分析,IGF-1表达水平及IGF-1/IGFBP-3比值对催乳素细胞腺瘤预后有较为优势且显著的预测价值,IGFBP-3表达水平对催乳素细胞腺瘤诊断无明显预测价值。结论IGF-1及IGF-1/IGFBP-3比值对于催乳素细胞腺瘤预后具有一定的预测价值,值得临床医师关注。Objective To analyze the expression of insulin-like growth factor 1(IGF-1)and insulin-like growth factor binding protein 3(IGFBP-3)in patients with prolactin-cell adenoma and their clinical significance.Methods A total of 60 patients with prolactin-cell adenoma admitted to the Second Affiliated Hospital of Kunming Medical University from January 2018 to December 2021 were selected as the study group,and 60 volunteers who underwent health check-ups during the same period were selected as the control group.The expression of IGF-1 and IGFBP-3 and the ratios of IGF-1/IGFBP-3 were compared between the study group and the control group.Results The IGF-1 expression level and IGF-1/IGFBP-3 ratio were higher in the study group than those in the control group,and the differences were all statistically significant(P<0.05),but no statistically significant difference in IGFBP-3 expression levels between the two groups(P>0.05).The differences in IGF-1 expression levels and IGF-1/IGFBP-3 ratio were not statistically significant(P>0.05)when compared patients with prolactin-cell adenoma of different genders,while IGF-1 expression levels and IGF-1/IGFBP-3 ratio were higher in patients with invasive tumours than those in patients with non-invasive tumours,both of which were statistically significant(P<0.05).The receiver operator characteristic(ROC)curve analysis showed that the IGF-1 expression level and IGF-1/IGFBP-3 ratio were superior and significant predictors of prognosis in prolactin-cell adenoma,while the IGFBP-3 expression level was not a significant predictor of prolactin-cell adenoma diagnosis.Conclusion The ratio of IGF-1 and IGF-1/IGFBP-3 can predict the prognosis of prolactin-cell adenoma,which is worth the attention of clinicians.
