摘　　要：目的:了解近年来河南省肺癌的发病特征和变化趋势。方法:收集2012至2018年河南省8家综合/肿瘤专科医院原发性肺癌病例资料,对不同特征患者的病理类型构成进行比较,采用Joinpoint回归计算各病理类型构成比的年度变化百分比(APC)。结果:共收集11 207例,其中男7 673例,女3 534例。发病年龄以60~69岁为主,病例占38.77%。腺癌、鳞状细胞癌(鳞癌)、小细胞肺癌和其他类型占比分别为52.34%、22.99%、19.51%和5.16%。2012至2018年肺腺癌占比明显上升(APC=3.11%,年龄调整APC=3.47%,P均<0.05),鳞癌占比呈下降趋势(APC=-6.60%,年龄调整APC=-7.57%,P均<0.05),小细胞肺癌(APC=-2.90%,年龄调整APC=-2.48%,P均>0.05)和其他类型肺癌(APC=9.27%,年龄调整APC=9.99%,P均>0.05)占比平稳波动。结论:近年来河南省肺腺癌占比呈上升趋势,鳞癌呈下降趋势;目前腺癌是肺癌最主要的病理类型。Aim:To understand the epidemic characteristics and trend of lung cancer in Henan Province in recent years.Methods:Clinical data of patients with primary lung cancer during 2012-2018 was collected from 8 general/tumor hospitals in Henan Province.The distribution of pathological subtypes was compared among patients with different clinical characteristics.Trends of pathological subtypes of lung cancer were analyzed with annual percentage change(APC)and APC adjusted by age(APCa)using Joinpoint regression.Results:A total of 11207 cases of primary lung cancer were collected,including 7673 males and 3534 females.The age of onset was mainly 60-69 years,which accounted for 38.77%.Adenocarcinoma(ADC),squamous cell carcinoma(SCC),small cell lung cancer(SCLC)and other pathological subtypes accounted for 52.34%,22.99%,19.51%and 5.16%.During 2012-2018,the proportion of ADC has increased(APC=3.11%,APCa=3.47%,P<0.05),whereas the proportion of SCC has decreased(APC=-6.60%,APCa=-7.57%,P<0.05);SCLC(APC=-2.90%,APCa=-2.48%,P>0.05)and other pathological subtypes(APC=9.27%,APCa=9.99%,P>0.05)showed stable trends.Conclusion:The proportion of ADC of lung cancer in Henan Province has increased in recent years,whereas SCC has decreased,and ADC is now the most common pathological subtype.
