改良Halterman矫治器治疗异位上颌第一恒磨牙的临床探讨

摘　　要：目的:探讨改良Halterman矫治器用于治疗异位上颌第一恒磨牙的临床疗效。方法:选取合肥市口腔医院就诊的异位上颌第一恒磨牙患者31名,共41颗患牙,应用改良Halterman矫治器对异位上颌第一恒磨牙进行矫治,测量全景片中异位上颌第一恒磨牙的嵌入量及分级程度。结果:异位上颌第一恒磨牙均正常萌出。Ⅱ、Ⅲ、Ⅳ级患侧第二乳磨牙分别在治疗后3、6、12个月的存留率差异均无统计学意义(P>0.05),而Ⅳ级患侧第二乳磨牙在治疗后12个月的留存率低于Ⅱ级患侧(P<0.05)。Ⅲ与Ⅳ级患牙嵌入量大于Ⅱ级患牙(P<0.05),而Ⅲ与Ⅳ级患牙嵌入量差异无统计学意义(P>0.05)。结论:应用改良Halterman矫治器治疗异位萌出的上颌第一恒磨牙效果良好。随着上颌第一恒磨牙异位萌出的级数升高,第一恒磨牙的嵌入量增大。Objective:To investigate the clinical efficacy of modified Halterman appliance in the treatment of ectopic eruption of the maxillary first molars.Methods:Thirty-one patients with ectopic eruption of maxillary first permanent molars(a total of 41 teeth)were included,and treated in our hospital by modified Halterman appliance.Panoramic radiograph was taken to measure the embedded amount and grade of ectopic eruption of the maxillary first molars.Results:Ectopic maxillary first permanent molars were normally erupted.There was no significant difference in the survival rate of gradeⅡ,Ⅲand IV second mammary molars at the involved side at 3,6 and 12 months after treatment(P>0.05),yet the retention rate of grade IV second primary molars at the affected side at 12 months after treatment was significantly lower than that of gradeⅡ(P<0.05).No difference was seen at 12 months after treatment regarding the retention rate of gradeⅡ,Ⅲ,and IV second primary molars(P<0.05).The embedded amount was significantly greater in gradeⅢand IV molars than in gradeⅡ,yet the difference was insignificant in gradeⅢand IV molars(P>0.05).Conclusion:Modified Halterman appliance can lead to better outcomes in the treatment of ectopic eruption of maxillaryfirst permanent molars,particularly added embedded amount with increase of the level of ectopic eruption of maxillary first permanent molars.
