摘 要:目的探讨双容积重建技术(double volume reconstruction technique,DVRT)在颅内动脉瘤介入栓塞术后随访中的应用价值。方法选取动脉瘤介入栓塞术的36例患者行常规脑血管造影(2D-DSA),对栓塞治疗的血管行三维数字减影血管造影(3D-DSA),并进行双容积重建,重点观察三者对栓塞动脉瘤残留的检出情况。结果36例患者中2D-DSA检查显示完全栓塞26例,瘤颈残留6例,瘤体残留4例;3D-DSA显示完全栓塞17例,瘤颈残留11例,瘤体残留8例;双容积重建图像显示完全栓塞15例,瘤颈残留12例,瘤体残留9例。2D-DSA与双容积重建对动脉瘤残留诊断的比较差异有统计学意义(Z=-2.54,P<0.05);3D-DSA与双容积重建比较差异无统计学意义(Z=-0.45,P>0.05)。结论DVRT可清晰显示出对比剂在弹簧圈中显影的程度以及位置,在评价弹簧圈栓塞的效果上能提供比2D-DSA、3D-DSA更丰富的影像学信息。Objective To explore the value of double volume reconstruction technique in follow-up of intracranial aneurysm with interventional treatment.Methods 36 patients with aneurysm embolization were examined with conventional cerebral angiography(2D-DSA),while the vascular with aneurysm embolization was conducted three-dimensional digital subtraction angiography(3D-DSA),and then double volume was reconstructed,the detection of aneurysm remnant was carefully observed.Results Of the 36 cases,2D-DSA did not show aneurysm remnant in 26 cases,aneurismal neck remnant was displayed in 6 cases,aneurismal body remnant was detected in 4 cases;3D-DSA did not show aneurysm remnant in 17 cases,aneurismal neck remnant was displayed in 11 cases,aneurismal body remnant was detected in 8 cases;double volume reconstruction image did not show aneurysm remnant in 15 cases,aneurismal neck remnant was displayed in 12 cases,aneurismal body remnant was detected in 9 cases.There was a statistical significance between 2D-DSA and double volume reconstruction image(Z=-2.54,P<0.05)when detecting an aneurysm remnant;and there was no significant difference between 3D-DSA and double volume reconstruction(Z=-0.45,P>0.05).Conclusion The double volume reconstruction technique can clearly show the degree and position of the contrast medium within the coils,which can provide much more imaging information than 2D-DSA and 3D-DSA in evaluating the embolization effectiveness.
