摘  要:目的利用磁共振弥散张量成像技术(DTI)探讨青少年原发性甲状腺功能亢进症(甲亢)患者白质神经纤维是否受到损害,并作相关性分析。方法选取32例青少年原发性甲亢患者(甲亢组)和32例健康志愿者(对照组)行全脑弥散张量成像序列扫描。结果青少年原发性甲亢患者颅脑双侧额叶、胼胝体压部局部区域各向异性分数(FA)值明显降低(P<0.01),甲亢组(双侧额叶)平均FA值为0.68±0.19,对照组FA值为0.77±0.13,P<0.05;甲亢组(胼胝体压部)平均ADC值为0.57±0.21,对照组平均ADC值为0.54±0.19,P>0.05。放射冠区FA值减低与甲亢组无明显相关性(R=-0.17,P=0.35)。甲亢组和对照组FA值ROC曲线AUC为0.75。结论DTI技术能够发现青少年原发性甲亢患者脑内微细结构病理变化,为青少年原发性甲亢患者的精神改变及病理生理机制提供一种非介入研究方法和研究依据。Objective Diffusion tensor magnetic resonance imaging(DTI)was used to investigate the damage of white matter nerve fibers in adolescent patients with primary hyperthyroidism.Methods Thirty-two adolescent patients with primary hyperthyroidism(hyperthyroid group)and 32 healthy volunteers(control group)were selected for whole-brain diffusion tensor imaging sequence scanning.Results In adolescent patients with primary hyperthyroidism,the FA value of bilateral frontal lobe and local area of corpus callosum was significantly decreased(P<0.01).The average FA value of the case group(bilateral frontal lobe)was 0.68±0.19,and that of the control group was 0.77±0.13,P<0.05.The average ADC value of the case group(corpus callosum pressure part)was 0.57±0.21,and the average ADC value of the control group was 0.54±0.19,P>0.05.There was no significant correlation between the reduction of FA value in the corona area and the case group(R=-0.17,P=0.35).The AUC of FA value ROC curve of adolescent primary hyperthyroidism group and healthy control group was 0.75.Conclusion DTI technology can detect the pathological changes of the fine structure in the brain of adolescent patients with primary hyperthyroidism,and provide a non-interventional research method for the study of the mental changes and pathophysiological mechanism of adolescent primary hyperthyroidism.
