摘　　要：目的探讨分析胰腺腺鳞癌的CT影像特征,提高对该病的认识。方法选取30例胰腺腺鳞癌患者的临床影像资料,包括临床症状、肿瘤标志物、病灶形态、强化方式、血管受累、邻近组织受侵和转移等。结果17例位于胰体尾,13例位于胰头。病灶平均直径(4.88±2.25)cm。病灶通常表现为边界模糊(56.7%)、形态不规则(86.7%)和囊性为主(66.7%)。增强后呈外周环形强化(80.0%)和中心渐进性强化(86.7%)。18例(60.0%)胰周受侵、9例(30.0%)血管受侵和14例(46.7%)淋巴结转移。结论胰腺腺鳞癌好发于胰腺体尾部,通常体积较大、形态不规则、囊性为主,增强扫描呈外周环形强化,中心渐进性强化。肿瘤易侵犯临近空腔脏器和血管,且较小时即可发生淋巴结转移。Objective To investigate the computed tomography(CT)imaging features of pancreatic adenosquamous carcinoma(PASC),so as to get a better understanding of this desease.Methods Clinical imaging data of 30 patients with PASC from June 2013 to November 2020 in our hospital were collected and analyzed,including clinical symptoms,tumor markers,tumor morphology,enhancement pattern,vascular involvement,adjacent tissue invasion and metastasis.Results 17 lesions were located in the tail/body of the pancreas and 13 in the head.The mean diameter of the lesions was(4.88±2.25)cm.The lesions were usually ill-defined(56.7%),irregular(86.7%),and predominantly cystic(66.7%).After enhancement,peripheral ring enhancement(80.0%)and central progressive enhancement(86.7%)were commonly seen.There were 18 cases(60.0%)of peripancreatic invasion,12 cases(40%)of vascular invasion and 14 cases(46.7%)of lymph node metastasis.Conclusion PASC occurs mostly in the body and tail of the pancreas.It tends to be a large,irregular and cystic mass with peripheral ring enhancement and progressive enhancement in the center.PASC is easy to invade adjacent cavity organs and blood vessels.Small PASC lesions can be associated with lymph node metastasis.
