摘　　要：目的 分析隧道成形术结合根向复位瓣术对Ⅲ度根分叉病变患者菌斑指数(PLI)、龈沟出血指数(SBI)和探诊深度(PD)的影响。方法 选取我院2018年1月至2021年1月收治的80例Ⅲ度根分叉病变患者为研究对象,按照电脑盲选法将其分为对照组(n=40,牙周基础治疗)与研究组(n=40,隧道成形术结合根向复位瓣术)。比较两组的治疗效果。结果 术后6个月,两组的PLI、SBI及PD均降低,且研究组低于对照组(P<0.05)。术后2个月,研究组的基质金属蛋白酶-9(MMP-9)、白细胞介素-6(IL-6)水平低于对照组,白细胞介素-2(IL-2)水平高于对照组(P<0.05)。研究组的治疗总有效率高于对照组,并发症总发生率低于对照组(P<0.05)。结论 隧道成形术结合根向复位瓣术治疗Ⅲ度根分叉病变的效果显著,有利于改善患者的牙周指标,减轻炎症反应,且并发症少。Objective To analyze the effects of tunnel plasty combined with apically repositioned flap surgery on plaque index(PLI),sulcus bleeding index(SBI)and probing depth(PD)in patients withⅢdegree furcation involvement.Methods Eighty patients withⅢdegree furcation involvement admitted in our hospital from January 2018 to January 2021 were selected as the research objects.According to the method of computer blind selection,the patients were divided into control group(n=40,periodontal basic treatment)and study group(n=40,tunnel plasty combined with apically repositioned flap surgery).The therapeutic effects of the two groups were compared.Results At 6 months after surgery,PLI,SBI and PD of both groups decreased,and those in the study group were lower than the control group(P<0.05).At 2 months after surgery,the levels of matrix metalloproteinase-9(MMP-9)and interleukin-6(IL-6)in the study group were lower than those in the control group,while the level of interleukin-2(IL-2)was higher than that in the control group(P<0.05).The total effective rate of treatment of the study group was higher than that of the control group,and the total incidence of complications was lower than that of the control group(P<0.05).Conclusion The tunnel plasty combined with apically repositioned flap surgery in the treatment ofⅢdegree furcation involvement has a significant effect,which is beneficial to improve the periodontal indexes of patients and reduce inflammatory response,with less complications.
