摘  要:目的检测乳腺癌组织信号转导及转录激活因子3(STAT3)、Fascin-1(FSCN1)蛋白表达,探讨STAT3与FSCN1蛋白表达的相关性及其临床病理意义。方法收集2007年1月至2019年6月本院病理科保存的浸润性乳腺癌组织标本411例和同时切除的正常乳腺组织标本40例。采用免疫组化法检测乳腺癌和乳腺正常组织STAT3和FSCN1蛋白表达,分析STAT3和FSCN1蛋白表达的相关性。237例乳腺癌患者获得了预后生存分析资料,将患者分为STAT3和FSCN1均强阳性组(n=14)、STAT3或FSCN1单一强阳性组(n=64)和STAT3和FSCN1均非强阳性组(n=159),Kaplan-Meier法绘制生存曲线。结果正常乳腺和乳腺癌组织中STAT3蛋白表达强阳性率分别为15.0%(6/40)和30.4%(125/411),组间比较差异有统计学意义(P=0.040)。年龄更小、组织学高级别、雌激素受体(ER)阴性、孕激素受体(PR)阴性和三阴性分子亚型者中,乳腺癌组织STAT3强阳性表达率明显更高(均P<0.05);人表皮生长因子受体2(HER2)阳性的乳腺癌患者STAT3强阳性表达率低于HER2非阳性患者(P<0.05)。乳腺癌组织中FSCN1蛋白强阳性率为19.3%(76/394)。STAT3强阳性表达患者的FSCN1蛋白强阳性率(30.7%,35/114)较STAT3非强阳性表达者(14.6%,41/280)显著增加,差异有统计学意义(P<0.001)。Spearman相关分析显示乳腺癌组织中STAT3和FSCN1蛋白表达显著正相关(rs=0.185,P<0.001)。生存分析显示STAT3和FSCN1均强阳性的乳腺癌患者5年平均生存期及5年总生存率显著低于STAT3或FSCN1单一强阳性患者及STAT3和FSCN1均非强阳性患者,差异有统计学意义(χ^(2)=6.45,P=0.011)。结论乳腺癌组织STAT3高表达并与FSCN1表达正相关,STAT3和FSCN1二者同时高表达与患者预后不良有关。Objective To investigate the protein expression of signal transducer and activator of transcription 3(STAT3)and Fascin-1(FSCN1)in breast cancer tissue,the correlation between STAT3 and FSCN1 expression,and their clinicopathological significance.Methods A total of 411 invasive breast cancer tissue samples and 40 concurrently-resected normal breast tissue samples,stored in the Department of Pathology,Dongyang People's Hospital between January 2007 and June 2019,were enrolled in this study.The protein expression of STAT3 and FSCN1 was examined using immunohistochemistry.The correlation between STAT3 and FSCN1 protein expression was analyzed.Prognostic data from 237 breast cancer patients were available for survival analysis.These patients were divided into the dual strong STAT3/FSCN1 positivity group(n=14),single strong STAT3/FSCN1 positivity group(n=64)and non strong STAT3/FSCN1 positivity group(n=159).Kaplan-Meier method was used to generate the survival curve.Results The strong-positive rate of STAT3 protein in normal breast tissue and breast cancer were 15.0%(6/40)and 30.4%(125/411),respectively,with statistically significant difference between groups(P=0.040).The strong-positive rate of STAT3 expression was significantly higher with younger age,higher histological grades,estrogen receptor(ER)or progesterone receptor(PR)negativity,and triple-negative breast cancer patients(all P<0.05).Patients with human epidermal growth factor receptor 2(HER2)positivity had a lower strong-positive rate of STAT3 expression than non-HER2-positive patients(P<0.05).The strong-positive rate of FSCN1 expression in breast cancer tissue specimens was 19.3%(76/394).The strong-positive rate of FSCN1 expression was significantly higher in patients with strong-positive STAT3 expression(30.7%,35/114)compared with patients who were not STAT3 strong-positive(14.6%,41/280),with statistically significant difference(P<0.001).Spearman correlation analysis revealed a significantly positive correlation between the protein expression of STAT3 and FS
