四川省精神科医师开展公共精神卫生服务能力现况及影响因素研究

摘　　要：目的 了解四川省精神科医师开展公共精神卫生服务的能力现况及影响因素。方法 按地区经济水平多阶分层抽样选择研究对象,利用经专家咨询及预调查的自制精神科医师公共精神卫生服务能力问卷收集信息。采用描述性统计分析方法、因子分析法、多重线性回归模型描述能力现况并探索其影响因素。结果 四川省精神科医师公共精神卫生服务能力理论知识模块得分率为72.27%(临床专业知识、政策管理知识、精神卫生基础保健知识维度得分率分别为79.00%、83.83%、57.33%)、技能掌握模块得分率为72%(沟通能力、工作执行能力维度得分率分别为70.10%、72.42%);其中涉及双向转诊、患者康复训练、危险性分级的条目得分率仅分别为5.2%、24%、57.33%。因子分析和多重线性回归结果显示,精神科医师人员稳定性好是理论得分(标准化β=0.279)和技能得分(标准化β=0.273)的共同促进因素,医师文化程度高是理论得分(标准化β=0.184)的促进因素。结论 四川省精神科医师参与公共精神卫生服务工作的能力普遍有待提高,人员稳定性有待加强,人才队伍结构尚需优化。Objective To investigate the current situation and influencing factors of psychiatrists’ability to provide public mental health services in Sichuan Province.Methods The subjects were selected by stratified sampling according to regional economic level,and the information was collected by using a self-made questionnaire on psychiatrists’public health service capacity based on expert consultation and pre-survey.Descriptive statistical analysis method,factor analysis method,and multiple linear regression model were used to describe the current situation of capability and explore the influencing factors.Results The scoring rate of theoretical knowledge module of public health service ability of psychiatrists in Sichuan Province was 72.27%,and the scoring rate of clinical professional knowledge,policy management knowledge,and mental health basic health knowledge were 79.00%,83.83%and 57.33%,respectively.The scoring rate of skill mastery module was 72%,and the scoring rate of communication ability and work execution ability were 70.10%and 72.42%,respectively.However,the scoring rates of items involving two-way referral,patient rehabilitation training,and risk classification were only 5.2%,24%,and 57.33%,respectively.The results of factor analysis and multiple linear regression showed that good stability of psychiatrists was the common contributing factor of theoretical score(standardizedβ=0.279)and skill score(standardizedβ=0.273),and high education level of psychiatrists was the contributing factor of theoretical score(standardizedβ=0.184).Conclusion The ability of psychiatrists in Sichuan Province to provide public mental health services generally needs to be improved,the stability of personnel needs to be strengthened,and the structure of talent team needs to be optimized.
