VRD-lite方案治疗老年多发性骨髓瘤的疗效

摘　　要：目的探讨硼替佐米+地塞米松+来那度胺(VRD-lite方案)治疗老年多发性骨髓瘤的疗效。方法将102例老年多发性骨髓瘤患者按照治疗方法不同分为对照组和观察组,每组51例。对照组采取硼替佐米+地塞米松方案治疗,观察组采取VRD-lite方案治疗。对比两组患者的临床疗效、免疫抑制因子水平、免疫功能指标、肾功能指标。结果治疗后,观察组患者总有效率高于对照组,差异有统计学意义(P﹤0.05)。治疗后,两组患者血清白细胞介素-6(IL-6)、白细胞介素-17(IL-17)、转化生长因子-β(TGF-β)、肌酐(Scr)、尿素氮(BUN)水平均降低,且观察组患者血清IL-6、IL-17、TGF-β、Scr、BUN水平均低于对照组,差异均有统计学意义(P﹤0.05)。治疗后,两组患者CD3^(+)、CD4^(+)水平及CD4^(+)/CD8^(+)均升高,CD8^(+)水平均降低,且观察组患者CD3^(+)、CD4^(+)水平及CD4^(+)/CD8^(+)均高于对照组,CD8^(+)水平低于对照组,差异均有统计学意义(P﹤0.05)。结论VRD-lite方案治疗老年多发性骨髓瘤可改善患者免疫功能及肾功能,提高治疗效果。Objective To investigate the efficacy of bortezomib+dexamethasone+lenalidomide(VRD-lite)regimen in the treatment of elderly patients with multiple myeloma.Method A total of 102 elderly patients with multiple myeloma were divided into the control group(n=51,treated with bortezomib+dexamethasone)and the observation group(n=51,treated with VRD-lite).The clinical efficacy,immunosuppressive factor levels,immune function indexes and renal func‐tion indexes were compared between the two groups.Result After treatment,the total effective rate of observation group was higher than that of control group(P<0.05);the serum levels of interleukin-6(IL-6),interleukin-17(IL-17),transforming growth factor-β(TGF-β),serum creatinine(Scr),blood urea nitrogen(BUN)in both groups decreased,with lower levels in the observation group than those in the control group(P<0.05).After treatment,the levels of CD3^(+),CD4^(+),and CD4^(+)/CD8^(+)increased and CD8^(+)decreased in both groups,with higher CD3^(+),CD4^(+),and CD4^(+)/CD8^(+)and lower CD8^(+)in the observation group than those in the control group(P<0.05).Conclusion VRD-lite regimen can improve the im‐mune function and renal function of elderly patients with multiple myeloma,and improve the therapeutic effect.
