泪道高频治疗仪联合RS泪道再通管对鼻泪管阻塞的治疗效果研究

摘　　要：目的:探讨泪道高频治疗仪联合RS泪道再通管对鼻泪管阻塞的治疗效果。方法:前瞻性选取2018年5月至2020年1月于某院接受治疗的鼻泪管阻塞患者158例,随机分为对照组(79例,82眼)和观察组(79例,85眼),分别进行疏通术联合药物治疗和泪道高频治疗仪联合RS泪道再通管治疗。比较2组基线资料、临床疗效、泪液弯液面水平、泪膜破裂时间、泪液分泌量、不良反应以及并发症情况。采用SPSS 16.0软件进行统计学分析。结果:观察组总有效率明显高于对照组(92.94%vs 71.95%,P<0.05)。2组治疗后各时间点泪液弯液面水平均较治疗前降低且观察组低于对照组(P<0.05)。组间以及组内不同时间点泪膜破裂时间差异无统计学意义(P>0.05)。观察组治疗1周、1个月、3个月后泪液分泌量高于对照组(P<0.05),治疗9个月后泪液分泌量低于对照组(P<0.05)。2组不良反应和并发症总发生率差异无统计学意义(χ^(2)=0.995,P=0.318)。结论:泪道高频治疗仪联合RS泪道再通管治疗鼻泪管阻塞能明显提高临床疗效、降低泪液弯液面水平、改善溢泪症状,同时不影响泪膜破裂时间,未增加不良反应和并发症,值得在临床上推广应用。Objective To investigate the therapeutic effect of the high frequency lacrimal duct therapeutic apparatus combined with RS lacrimal tube on nasolacrimal duct obstruction.Methods Totally 158 nasolacrimal duct obstruction patients treated at some hospital from May 2018 to January 2020 were selected prospectively and randomly divided into a control group(79 cases,82 eyes)and an observation group(79 cases,85 eyes),who underwent the treatment by dredging surgery combined with medication and high-frequency lacrimal duct therapeutic apparatus combined with RS lacrimal tube,respectively.The baseline data,clinical efficacy,tear meniscus level,tear break-up time,tear secretion,adverse effects and complications were compared between the 2 groups.SPSS 16.0 software was used for statistical analysis.Results The total effective rate of the observation group was significantly higher than that of the control group(92.94%vs 71.95%,P<0.05).The two groups both had the tear meniscus levels dropped when compared with those before treatment,and the level in the observation group was lower than that in the control group(P<0.05).The inter-and intra-group differences between the tear break-up time were not statistically significant(P>0.05).The tear secretion in the observation group was higher 1 week,1 month and 3 months after treatment(P<0.05)while lower than that in the control group 9 months after treatment(P<0.05).There were no statistically significant differences between the overall incidence rates of the adverse effects and complications in the 2 groups(χ^(2)=0.995,P=0.318).Conclusion When used for treating nasolacrimal duct obstruction the high-frequency lacrimal duct therapeutic apparatus combined with RS lacrimal tube significantly enhances the clinical efficacy,lowers the tear meni-scus level and improves the symptoms of epiphora while has no influence on tear break-up time and does not result in adverse effects and complications,and thus is worthy promoting clinically.
