EC-wP、EC-T和TEC新辅助化疗方案治疗乳腺癌的疗效比较

摘　　要：目的探讨表柔比星+环磷酰胺序贯紫杉醇(EC-wP)方案、表柔比星+环磷酰胺序贯多西他赛(EC-T)方案及多西他赛+表柔比星+环磷酰胺(TEC)方案治疗乳腺癌的疗效。方法将90例乳腺癌患者根据化疗方案不同分为EC-wP组、EC-T组、TEC组,每组30例。比较3组患者的治疗总有效率、预后、血清肿瘤标志物[癌胚抗原(CEA)、糖类抗原125(CA125)、糖类抗原153(CA153)、基质金属蛋白酶9(MMP9)]水平及不良反应发生情况。结果3组患者的治疗总有效率比较,差异无统计学意义(P>0.05)。EC-wP组患者3年无病生存率和总生存率均高于EC-T组和TEC组患者,差异均有统计学意义(P<0.05)。化疗后,3组患者各血清肿瘤标志物水平均较化疗前降低,EC-wP组和EC-T组患者各血清肿瘤标志物水平均低于TEC组,EC-wP组患者各血清肿瘤标志物水平均低于EC-T组,差异均有统计学意义(P<0.05)。EC-wP组和EC-T组患者中性粒细胞减少发生率均低于TEC组,EC-wP组患者中性粒细胞减少发生率低于EC-T组,差异均有统计学意义(P<0.05)。结论EC-wP、EC-T、TEC化疗方案在乳腺癌治疗中均具有良好的效果,但相较于EC-T、TEC化疗方案,EC-wP化疗方案的安全性较高,治疗效果较为显著,治疗后可有效抑制肿瘤的生长。Objective To investigate the efficacy of epirubicin+cyclophosphamide sequential paclitaxel(EC-wP)regimen,epirubicin+cyclophosphamide sequential docetaxel(EC-T)regimen and docetaxel+epirubicin+cyclophosphamide(TEC)regimen in the treatment of breast cancer.Method A total of 90 breast cancer patients were divided into EC-wP group,EC-T group and TEC group according to different chemotherapy regimens,with 30 cases in each group.The total effective rate,prognosis,serum tumor markers[carcinoembryonic antigen(CEA),carbohydrate antigen 125(CA125),carbohydrate antigen 153(CA153),matrix metalloproteinase 9(MMP9)]levels and adverse events of the three groups were compared.Result The total effective rates of treatment were not statistically significant between the three groups(P>0.05).The 3-year disease-free survival and overall survival of patients in the EC-wP group were higher than those in the EC-T group and TEC group,and the differences were statistically significant(P<0.05).After the chemotherapy,the serum tumor marker levels in the three groups were lower than those before the chemotherapy,the serum tumor marker levels in the EC-wP group and the EC-T group were lower than those in the TEC group,and the serum tumor marker levels in the EC-wP group were lower than those in the EC-T group,and the differences were statistically significant(P<0.05).The incidence of neutropenia in the EC-wP group and the EC-T group were lower than that in the TEC group,the incidence of neutropenia in the EC-wP group was lower than that in the EC-T group,and the differences were statistically significant(P<0.05).Conclusion The EC-wP,EC-T,and TEC chemotherapy regimens have good effects in the treatment of breast cancer.Attentionally,compared with EC-T and TEC chemotherapy regimens,EC-wP chemotherapy regimens have higher safety and significant treatment effects,and can effectively inhibit tumor growth after treatment.
