胃腺癌组织中SIRT-1及PD-L1蛋白的表达及临床意义

摘　　要：目的 探讨胃腺癌组织中沉默信息调节因子1(SIRT-1)及程序性死亡配体1(PD-L1)蛋白的表达及临床意义。方法 选取邯郸市中心医院2017年1月至2021年1月收治的53例胃腺癌患者为研究对象,收集患者手术切除的胃腺癌组织及癌旁正常组织标本。采用HE染色法观察并比较胃腺癌细胞与癌旁正常胃黏膜上皮细胞形态学,采用免疫组化法检测并比较胃腺癌组织及癌旁正常组织SIRT-1、PD-L1蛋白表达,分析胃腺癌患者临床病理特征与SIRT-1、PD-L1蛋白阳性表达率的关系,以及SIRT-1与PD-L1蛋白表达的相关性。结果 HE染色可见胃癌细胞形态及大小不一,分化差,细胞核大,胞质少,核分裂象明显;而癌旁正常胃黏膜上皮细胞分布均匀,腺体明显,细胞核较小,胞质丰富,未见明显核分裂。胃腺癌组织SIRT-1、PD-L1蛋白阳性表达率均明显高于癌旁正常组织(60.4%比32.1%,66.0%比35.8%,均P<0.05)。肿瘤浸润较深、TNM分期Ⅲ~Ⅳ期、存在淋巴结转移的患者胃腺癌组织中SIRT-1、PD-L1蛋白阳性表达率均明显较高(均P<0.05);而不同性别、年龄、肿瘤直径及肿瘤部位患者胃腺癌组织中SIRT-1、PD-L1蛋白阳性表达率比较,差异均无统计学意义(均P>0.05)。胃腺癌组织SIRT-1与PD-L1蛋白表达呈正相关(rs=0.666,P<0.05)。结论 胃腺癌组织中SIRT-1及PD-L1蛋白表达均较高,两者可能在胃腺癌细胞增殖、侵袭和转移中存在协同作用。Objective To investigate the expression of silence signal regulating factor 1(SIRT-1)and programmed death ligand 1(PD-L1)in gastric adenocarcinoma and its clinical significance.Methods The cancer tissue and corresponding normal tissue specimens of 53 patients with gastric adenocarcinoma in Handan Central Hospital from January 2017 to January 2021 were collected.Immunohistochemical method was used to detect the expression of SIRT-1 and PD-L1 proteins in gastric cancer tissues and corresponding normal tissue.The relationship between expression of SIRT-1,PD-L1 and clinicopathological features of gastric cancer was analyzed,and the correlation between SIRT-1 and PD-L1 expression was also analyzed.Results The positive expression rates of SIRT-1 and PD-L1 protein in gastric cancer were significantly higher than those in adjacent normal tissues(60.4%vs.32.1%,66.0%vs.35.8%,P<0.05).The positive rates of SIRT-1 and PD-L1 expression were significantly higher in gastric cancer with deep infiltration,TNM Stage III-IV and lymph node metastasis(P<0.05),there were no significant differences in positive rates of SIRT-1 and PD-L1 expression among patients with different sex,age,tumor diameter and tumor location(P>0.05).There was a positive correlation between SIRT-1 and PD-L1 protein expression in gastric caner tissue(rs=0.666,P<0.05).Conclusion The expression of SIRT-1 and PD-L1 is up-regulated in gastric adenocarcinoma,suggesting that both proteins may be involved in the proliferation,invasion and metastasis of gastric cancer.
