摘　　要：目的分析帕金森病(PD)患者元音发声的共振峰频率变化及其与疾病严重程度的关系。方法本研究招募了83例PD患者和45名健康对照者进行元音“a”“o”“e”发声测试,并通过PRAAT软件对音频进行共振峰频率F1~F4提取。PD患者的疾病严重程度采用Hoehn-Yahr量表(H&Y)评估PD临床病情程度分级,统一帕金森病评定量表运动评分量表Ⅲ(UPDRSⅢ)评估PD患者的运动功能。比较PD患者与对照组元音“a”“o”“e”的共振峰频率F1~F4的差异,并分析PD患者元音共振峰频率与疾病严重程度的相关性。结果男女PD患者“a”“o”和“e”元音的共振峰频率F1~F4均显著低于对照组(均P<0.001)。PD男性患者“a”元音共振峰频率F1、F3和F4与H&Y评分和UPDRSⅢ评分均呈负相关,F2与UPDRSⅢ评分呈正相关;“o”“e”元音共振峰频率F1~F4与H&Y及UPDRSⅢ评分均呈负相关(均P<0.001)。PD女性患者除“o”元音F2与H&Y评分无相关性外,其余F1~F4共振峰频率与H&Y及UPDRS评分均呈显著负相关(均P<0.001)。结论PD患者“a”“o”“e”元音发音的共振峰频率F1~F4降低,并且其中绝大多数随着疾病的严重程度增加而降低。Objective The formant frequency of vowel sound in patients with Parkinson s disease(PD)and its relationship with disease severity were analyzed.Methods This study recruited 83 PD patients and 45 healthy controls for vowel("a""o""e")vocalization tests.The formant frequency was extracted by PRAAT software.Hoehn-Yahr Scale(H&Y)was used to evaluate the clinical severity of PD patients,and the motor function of PD patients was evaluated with the unified Parkinson s Disease Rating Scale Exercise Score partⅢ(UPDRSⅢ)scale.The differences of F1-F4 formant frequencies of vowel"a""o"and"e"in PD patients and control group were compared,and the correlation between formant frequencies and disease severity in PD patients was analyzed.Results Formant frequencies of"a""o"and"e"vowels in male and female PD patients were significantly lower than those in the control group(P<0.001).The formant frequencies F1,F3 and F4 of"a"vowels in male PD patients were negatively correlated with H&Y score and UPDRSⅢscore,while F2 was positively correlated with UPDRSⅢscore.The formant frequencies of"o"and"e"vowels F1 to F4 were negatively correlated with H&Y and UPDRSⅢscores(all P<0.001).In female PD patients,there was a significant negative correlation between the formant frequencies of F1-F4 and H&Y and UPDRS scores(P<0.001 for both),except for the"o"vowel F2 and H&Y scores.Conclusions The formant frequency of vowel sounds in PD patients decreased and most of them decreased with the increase of disease severity.
