摘  要:目的:针对罹患2型糖尿病的慢性牙周炎患者,开展牙周基础治疗后口服辅酶Q_(10)3个月,观察其牙周临床指数、龈沟内炎症因子及机体糖代谢水平的变化情况。方法:选取60例患有2型糖尿病的慢性牙周炎患者,随机将其分为观察组、对照组,每组30例。对照组仅实施常规牙周基础治疗,观察组在其基础上辅助口服辅酶Q_(10)3个月。两组均在完成各自治疗计划后第6个月复诊,比较牙周临床指数(PD、AL和SBI)、龈沟内炎症因子(IL-1β、IL-6)水平和糖代谢(FBG、HbA1c)水平。结果:(1)治疗前,两组PD、AL和SBI水平无统计学差异(P>0.05);复诊时,两组PD、AL、SBI均较治疗前显著降低(P<0.05),且观察组较对照组更低(P<0.05)。(2)治疗前,两组龈沟内IL-1β和IL-6的浓度无统计学差异(P>0.05);复诊时,两组IL-1β、IL-6均较治疗前降低(P<0.05),且观察组低于对照组(P<0.05)。(3)治疗前,两组FBG、HbA1c均无统计学差异(P>0.05);复诊时,两组FBG、HbA1c均显著低于治疗前(P<0.05),且观察组低于对照组(P<0.05)。结论:相较于单纯的牙周基础治疗,联合口服辅酶Q_(10)对于2型糖尿病的慢性牙周炎患者牙周临床指数、龈沟内炎症因子水平和机体糖代谢情况的控制和改善更显著有效。Objective:To investigate the effects of coenzyme Q_(10) combined with periodontal therapy on periodontal index,inflammatory factors and glucose metabolism in patients with type 2 diabetes mellitus complicated with chronic periodontitis.Methods:60 patients with type 2 diabetes accompanied by chronic periodontitis were selected,randomly divided into observation group and control group,30 cases per group.The control group received basic periodontal treatment,and the observation group took coenzyme Q_(10) orally for 3 months.Review at the 6th month after treatment,the periodontal clinical index(PD,AL and SBI)and inflammatory factor in gingival sulcus(IL-1β,IL-6)were compared between the two groups and glucose metabolism(FBG,HbA1c).Results:(1)There was no significant difference in PD,AL and SBI between the two groups before treatment(P>0.05).At the time of follow-up consultation,the PD,AL and SBI of the two groups were lower than those before treatment(P<0.05),and the PD,AL and SBI of the observation group were significantly lower than those of the control group(P<0.05).(2)Before treatment,IL-1β and IL-6 in gingival crevicular fluid of the two groups had no significant difference(P>0.05).At the time of follow-up consultation,IL-1βand IL-6 in both groups were lower than those before treatment(P<0.05),and IL-1β,IL-6 in the observation group were lower than those in the control group(P<0.05).(3)Before treatment,there was no significant difference in FBG and HbA1c between the two groups(P>0.05).At the time of follow-up consultation,the FBG,HbA1c decreased significantly in both groups(P<0.05),and FBG,HbA1c in the observation group were lower than those in the control group(P<0.05).Conclusion:Compared with simple periodontal treatment,coenzyme Q_(10) combined with periodontal basic therapy can more effectively control periodontal clinical index in type 2 diabetes mellitus patients with chronic periodontitis,reduce inflammatory factor levels and glucose metabolism level in gingival sulcus.
