摘 要:目的:利用扇形束螺旋CT系统分析讨论头颈部肿瘤影像引导放射治疗的摆位误差。方法:选取2020年8月至2021年8月,于我院接受影像引导放射治疗的头颈部肿瘤患者128例。分别扫描患者初步摆位后、误差纠正后、治疗结束后的CT图像,将治疗图像与计划图像匹配,获得患者肿瘤靶区中心点在X(左右)、Y(头脚)、Z(腹背)方向上的线性摆位误差,分析误差并讨论。结果:经统计总体线性误差在X、Y、Z方向上平均值分别为(0.91±1.33)、(0.97±1.59)、(0.87±1.28)mm。其中,初步摆位后误差数值分别为(0.98±2.08)、(0.96±2.42)、(0.83±1.79)mm,纠正后的摆位误差分别为(0.11±0.57)、(0.06±0.62)、(0.05±0.75)mm,与初步摆位后误差比较差异有统计学意义(P<0.05)。治疗结束后的摆位误差分别为(0.20±0.86)、(0.25±0.78)、(0.14±0.98)mm,与纠正后的摆位误差比较差异无统计学意义(P>0.05)。结论:在头颈部肿瘤放疗中,应用影像引导放射治疗技术纠正摆位误差可以使治疗个体化,为精确放疗提供了保证,具有推广价值。Objective:To investigate the positioning errors in image-guided radiation therapy(IGRT)for head and neck tumors using fan-shaped beam spiral CT system.Methods:From Aug 2020 to Aug 2021,128 patients with head and neck tumor treated with IGRT in our hospital were selected.The Siemens CT vision image-guided radiotherapy system was used to read the CT images scanned after the patient's initial positioning,error correction and post-treatment,and then the CT images were registered with original plan CT images,so as to obtain the linear positioning errors of X(left and right),Y(head and foot),Z(abdomen and back)in the three-dimensional direction of the patient's body center point.The errors were analyzed.Results:The average linear errors in X,Y and Z directions were(0.91±1.33),(0.97±1.59)and(0.87±1.28)mm.The error values after the initial positioning were(0.98±2.08),(0.96±2.42)and(0.83±1.79)mm,respectively.After correction,the positioning errors were(0.11±0.57),(0.06±0.62),(0.05±0.75)mm,respectively.The difference was statistically significant(P<0.05).The positioning errors after treatment were(0.20±0.86),(0.25±0.78)and(0.14±0.98)mm,respectively,and there was no statistically significant difference compared with the position errors after correction(P>0.05).Conclusions:In radiotherapy of head and neck tumors,utilizing IGRT technology to correct the positioning error can make the treatment more accurate and personalized.IGRT provides assurance for accurate delivery of radiotherapy,which has great significance and application value.
