皮肤鳞状细胞癌组织中miR-421、PDCD4 mRNA的表达及意义

摘　　要：目的探讨皮肤鳞状细胞癌(CSCC)组织中微小RNA(miR)-421、程序性细胞死亡因子4(PDCD4)mRNA的表达及临床意义。方法选取CSCC患者78例,取癌组织、癌旁组织(距癌组织边缘5 cm以上),以同期50例正常皮肤组织作为对照。应用荧光定量PCR法检测癌、癌旁及正常组织中miR-421、PDCD4 mRNA表达。相关性分析采用Pearson相关分析。应用miRNA数据库TargetScan对miR-421与PDCD4 mRNA可能的结合位点进行预测。比较miR-421、PDCD4 mRNA表达与CSCC患者临床病理特征的关系。采用受试者工作特征(ROC)曲线分析miR-421、PDCD4 mRNA及两者联合对CSCC发生的诊断价值。结果CSCC组织中miR-421表达高于癌旁组织和正常组织,PDCD4 mRNA表达低于癌旁组织和正常组织(P均<0.05)。CSCC组织中miR-421与PDCD4 mRNA表达呈负相关(r=-0.694,P<0.05)。TargetScan分析发现,PDCD4 mRNA的3´UTR区第419~425位点存在与miR-421结合的位点。不同肿瘤TNM分期、淋巴结转移CSCC患者癌组织中miR-421、PDCD4 mRNA表达差异有统计学意义(P均<0.05)。miR-421、PDCD4 mRNA及两指标联合诊断CSCC发生的曲线下面积分别为0.826、0.764、0.889,联合检测时曲线下面积优于miR-421、PDCD4 mRNA单项检测的诊断效能(Z分别为2.409、4.363,P均<0.05)。结论CSCC组织中miR-421表达升高,PDCD4 mRNA表达降低,两者与CSCC肿瘤TNM分期、淋巴结转移有关,二者联合检测有助于评估CSCC的发生。Objective To analyze the expression and clinical significance of microRNA-421(miR-421)and programmed cell death factor 4(PDCD4)mRNA in cutaneous squamous-cell carcinoma(CSCC).Methods Seventy-eight patients with CSCC were selected,and the cancerous tissues and paracancerous tissues(more than 5 cm from the edge of the cancerous tissues)were taken,and 50 normal skin tissues in the same period were taken as controls.The expression of miR-421 and PDCD4 mRNA was detected by fluorescence quantitative PCR.Pearson correlation analysis was used for correlation analysis.TargetScan,a miRNA database,was used to predict the possible binding sites of miR-421 and PDCD4 mRNA.The relationships between miR-421,PDCD4 mRNA expression and clinical pathological characteristics of CSCC patients were compared.The predictive value of miR-421,PDCD4 mRNA and their combination on the occurrence of CSCC was analyzed by receiver operating curve.Results The expression of miR-421 in CSCC cancer tissues was higher than that in the paracancerous and normal tissues,and the expression of PDCD4 mRNA was lower than that in the paracancerous and normal tissues(all P<0.05).There was a negative correlation between miR-421 and PDCD4 mRNA expression in CSCC tissues(r=-0.694,P<0.05).TargetScan analysis showed that the 419-425 sites in the 3´UTR region of PDCD4 mRNA had a binding site with miR-421.The expression of miR-421 and PDCD4 mRNA in CSCC patients with different tumor TNM stages and lymph node metastasis was significantly different(P<0.05).The area under the curve of miR-421,PDCD4 mRNA and the combination of the two indicators in the diagnosis of CSCC was 0.826,0.764 and 0.889,respec-tively.The area under the curve of the combined detection was better than that of either alone(Z=2.409 and 4.363,re-spectively;both P<0.05).Conclusions The increased expression of miR-421 and the decreased expression of PDCD4 mRNA in CSCC cancer are related to TNM stage and lymph node metastasis of CSCC.The combined detection of miR-421 and PDCD4 is helpful to predict th
