以认知障碍为首发症状蝶骨嵴脑膜瘤1例并文献复习

摘　　要：脑膜瘤是最常见的原发性中枢神经系统肿瘤,约占所有良性脑肿瘤的50%。脑膜瘤起源于大脑或脊髓的蛛网膜冒状细胞。临床表现取决于脑膜瘤的位置和大小。一些患者可能无症状,其他患者会出现典型神经功能损伤症状,如肢体活动障碍、嗅觉丧失、视野缺损、失语和癫痫发作等。全外科手术切除是治疗脑膜瘤的标准方式,其可以提供90%的5年无进展生存率。而相关文献报道有些脑膜瘤患者却出现了与上诉症状不相符的非典型症状,造成漏诊、误诊,延误治疗时机,降低预后。复习相关文献,现结合近期江汉大学附属武汉市中心医院收治的1例以认知障碍为首发症状脑膜瘤患者治疗情况,进行分析与总结。Meningioma is the commonest primary central nervous system tumor,accounting for about 50%of all benign brain tumors.Meningiomas originate from arachnoid papillary cells in the brain or spinal cord,with their clinical manifestations depending on the location and size of meningioma.Some patients may be asymptomatic,while others may have typical neurological impairment symptoms,such as limb movement disorder,olfactory loss,visual field defect,aphasia and seizures.Total surgical resection is the standard way to treat meningioma,which can provide 90%of 5-year progression-free survival rate.However,some related literatures reported that some patients with meningioma had atypical symptoms which were inconsistent with the above-mentioned symptoms,resulting in missed diagnosis and misdiagnosis,delaying the treatment opportunity and reducing the prognosis.With relevant literatures reviewed,and the treatment of a meningioma patient with cognitive impairment as the first symptom recently admitted to the Central Hospital of Wuhan Affiliated to Jianghan University as a case,analysis and summary are made accordingly in this paper.
