家庭韧性在化疗多发性骨髓瘤患者恐惧疾病进展、SSRS评分间的中介作用

摘　　要：目的分析家庭韧性在化疗多发性骨髓瘤(MM)患者恐惧疾病进展、社会支持评定量表(SSRS)评分间的中介作用。方法选取河南省濮阳市安阳地区医院2020年1月至2022年5月收治的化疗MM患者115例作为研究对象,利用家庭坚韧性量表(FHI)、SSRS、恐惧疾病进展简化量表(FoP-Q-SF)开展调查,通过AMOS 22.0构造结构方程模型,分析PICC置管化疗患者的家庭韧性于恐惧疾病进展以及社会支持之间的中介作用。结果FoP-Q-SF总分值为(36.08±5.58)分,整体处于心理失调状况;FHI总分值为(57.88±6.94)分,各维度得分由高到低依次为承担、调整与控制;SSRS总分值为(40.62±7.75)分,整体处于良好社会支持程度的状况;经Pearson分析可知,恐惧疾病进展与社会支持、家庭韧性呈负相关,家庭韧性与社会支持呈正相关(P<0.05);模型指出,家庭韧性于社会支持、恐惧疾病进展之间具有中介效应(P<0.05)。结论化疗MM患者的恐惧疾病进展整体处在心理失调状况,而家庭韧性与社会支持程度良好,且家庭韧性于恐惧疾病进展、社会支持程度之间起到中介效应,临床可针对家庭韧性实施干预,以改善恐惧疾病进展状况。Objective To analyze the mediating effect of family resilience on fear of disease progression and social support rating scale(SSRS)scores in patients with multiple myeloma(MM)treated with chemotherapy.Methods A total of 115 MM patients with chemotherapy admitted to Anyang District Hospital of Puyang City,Henan Province from January 2020 to May 2022 were selected as research objects.The family resilience index(FHI),SSRS,and fear of progression questionnaire-short form(FoP-Q-SF)were used to investigate.Structural equation model was constructed with AMOS 22.0.The mediating role of family resilience in fear of disease progression and social support in patients with PICC catheterized chemotherapy was analyzed.Results The total score of FoP-Q-SF was(36.08±5.58),which was in a state of psychological disorder;The total score of FHI was(57.88±6.94)points,and the score of each dimension was commitment,adjustment and control in order from high to low.The total score of SSRS was(40.62±7.75),indicating a good level of social support.Pearson analysis showed that the fear of disease progression was negatively correlated with social support and family resilience,while family resilience was positively correlated with social support(P<0.05).The model indicated that family resilience had a mediating effect between social support and fear of disease progression(P<0.05).Conclusion The fear of disease progression in MM patients with chemotherapy is generally in psychological disorder,and family resilience and social support are good,and family resilience plays an intermediary effect between the fear of disease progression and social support.Clinical intervention can be carried out to improve the fear of disease progression.
