摘　　要：目的 探索Damon系统配合种植支抗钉与使用Damon系统配合改良式Nance弓(横腭杆+腭托)两种不同方法 矫治重度牙列拥挤拔牙病例的效果。方法 选取2018年1月至2020年12月镇江市第一人民医院口腔科收治的40例重度牙列拥挤并拔除第一前磨牙矫治的患者作为研究对象,按照随机数字表法分为试验组(20例)与对照组(20例),试验组使用种植体支抗辅助治疗,对照组采用改良式Nance弓进行辅助,比较两组患者矫治前、矫治后、排齐整平后三个阶段上下中切牙位置角度,并比较两组患者治疗时间。结果 试验组矫治前后、排齐整平后三个阶段上下颌中切牙倾斜度比较,差异无统计学意义(P>0.05),总疗程为(13.60±3.50)个月;而对照组排齐整平后U1-SN角、L1-MP角、U1-NA角、U1-NA距、L1-NB角、L1-NB距、U1-L1角数据与矫治前比较,差异有统计学意义(P<0.05),总疗程为(17.10±2.65)个月。试验组治疗时间短于对照组,差异有统计学意义(P<0.01)。结论Damon系统配合种植体支抗的使用,能较好地对重度牙列拥挤拔牙病例进行高效矫治,避免矫治过程中前牙的往复移动,显著缩短矫治疗程。Objective To explore the effect of Damon system combined with implant anchorage and Damon system combined with modified Nance arch(transpalatal arch combined with palatal plate)in the treatment of severe dentition crowding cases treated by extraction.Methods A total of 40 patients with severe dentition crowding and extraction of the first premolars in Department of Stomatology,Zhenjiang First People's Hospital from January 2018 to December 2020 were selected as the research subjects,they were divided into the experimental group(20 cases)and the control group(20 cases)according to the random number table method.The experimental group used implant anchorage as an auxiliary appliance,and the control group used modified Nance arch as an auxiliary appliance.Compare the position and angles of upper and lower central incisors in two groups at three stages:pre and post treatment,after alignment and leveling.And compare the treatment time of two groups.Results There were no statistically significant differences in the inclination of upper and lower central incisors in the experimental group before and after treatment and after alignment and leveling(P>0.05),the total course of treatment was(13.60±3.50)months.However,compared with those before treatment,the data of U1-SN angle,L1-MP angle,U1-NA angle,U1-NA distance,L1-NB angle,L1-NB distance and U1-L1 angle in the control group after alignment and leveling were statistically significant(P<0.05),and the total course of treatment was(17.10±2.65)months.The treatment time of the experimental group was shorter than that of the control group,and the difference was statistically significant(P<0.01).Conclusion Damon system combined with the use of implant anchorage can effectively correct the extraction cases with severe dentition crowding,avoid the reciprocating movement of anterior teeth during the treatment process,and successfully shorten the treatment time.
