基于“扶补托毒”理论探讨黄芪甲苷的足细胞保护机制

摘　　要：黄芪甲苷是黄芪中的三萜类化合物之一,具有保护肾脏、抗炎症、抗氧化、抗细胞凋亡、降糖、降脂等作用,尤其是对足细胞的保护作用突出。从西医学而言,黄芪甲苷能稳定足细胞蛋白骨架,抗相关炎症因子、氧化自由基、凋亡分子,改善足细胞线粒体功能和抑制内质网应激,延缓足细胞上皮间-充质转化。从中医学理论而言,黄芪具备“扶补”“托毒”的功效,“扶补”体现在改善线粒体能量代谢、提高膜电位等方面,“托毒”体现在抑制氧化炎症反应、下调内质网应激、延缓足细胞上皮间-充质转化进展等方面。黄芪治疗肾病在中西医层面存在诸多生理病理的联系,本文基于“扶补托毒”理论,探讨黄芪甲苷保护足细胞的机制。Astragaloside IV is a triterpenoid compound derived from Astragalus membranaceus,which can play a significant role in kidney protection,anti-inflammation,anti-oxidation,anti-apoptosis,reducing blood sugar and reducing lipid,especially in the protection of podocytes.Astragaloside IV can stabilize podocyte protein skeleton,clear inflammatory factors,oxidative free radicals and apoptotic molecules,improve mitochondrial function of podocytes,inhibit endoplasmic reticulum stress,and delay epithelial-mesenchymal transition(EMT)of podocytes from modern medicine point of view.According to the theory of TCM,Astragalus membranaceus has the functions of‘tonifying’and‘detoxifying’.‘Tonifying’is reflected in improving mitochondrial energy metabolism and increasing membrane potential,and‘detoxifying’is reflected in inhibiting oxidative inflammatory response,down-regulating endoplasmic reticulum stress,and delaying EMT progression.There are many physiological and pathological connections between TCM and Western medicine in treatment of kidney disease with Astragalus membranaceus.This study intends to study the mechanism of Astragaloside IV in protecting podocytes based on the theory of‘tonifying and detoxifying’.
