颌间牵引钉联合微型钛板内固定在颌骨骨折治疗中的应用价值分析

摘　　要：目的 分析颌间牵引钉联合微型钛板内固定对颌骨骨折的治疗效果。方法 选取2017年1月—2021年12月江苏大学附属句容医院口腔科收治的50例颌骨骨折患者,根据患者入院就诊顺序分组。对照组25例行微型钛板内固定治疗,观察组25例在对照组基础上联合颌间牵引钉,对比两组治疗优良率、手术前后牙周指数和颌面部功能。结果 观察组优良率为96.00%,高于对照组的68.00%,差异有统计学意义(χ^(2)=4.878,P<0.05);两组术前牙周指数和颌面部功能对比,差异无统计学意义(P>0.05);观察组术后菌斑指数、软垢指数、牙龈指数、颌面部功能、咀嚼功能和开口功能评分分别为(0.98±0.16)分、(0.80±0.16)分、(1.24±0.21)分、(0.96±0.12)分、(1.02±0.21)分和(0.44±0.08)分,均低于对照组,差异有统计学意义(t=10.515、7.418、8.771、8.321、6.286、20.108,P<0.05)。结论 予以颌骨骨折患者颌间牵引钉联合微型钛板内固定方案,可提高临床疗效,对牙周指数和颌面部功能均有改善作用。Objective To analyze the therapeutic effect of intermaxillary distraction nail combined with miniature titanium plate internal fixation on jaw fracture.Methods 50 patients with jaw fractures admitted to the Department of Stomatology in the Jurong Hospital Affiliated to Jiangsu University from January 2017 to December 2021 were selected and grouped according to the order of admission.25 cases in the control group were treated with miniature titanium plate internal fixation,and 25 cases in the observation group were treated with intermaxillary traction nail on the basis of the control group.The excellent and good rate of treatment,periodontal index and maxillofacial function before and after operation were compared between the two groups.Results The excellent and good rate of the observation group was 96.00%,which was higher than 68.00%of the control group,and the difference was statistically significant(χ^(2)=4.878,P<0.05).There was no statistically significant difference in periodontal index and maxillofacial function between the two groups before surgery(P>0.05);the postoperative plaque index,soft scale index,gingival index,maxillofacial function,chewing function and opening function scores of the observation group were(0.98±0.16)points,(0.80±0.16)points,(1.24±0.21)points,(0.96±0.12)points,(1.02±0.21)points and(0.44±0.08)points,respectively,which were lower than those of the control group,the difference was statistically significant(t=10.515,7.418,8.771,8.321,6.286,20.108,P<0.05).Conclusion The intermaxillary distraction nail combined with miniature titanium plate internal fixation for patients with jaw fractures can improve the clinical efficacy.The effect on periodontal index and maxillofacial function was improved,and can be promoted.
