高频电凝下金属夹内镜下结肠黏膜切除术,氩等离子凝固术在结肠息肉中的治疗价值

摘　　要：目的 对比高频电凝下金属夹内镜下结肠黏膜切除术和氩等离子凝固术对结直肠息肉的治疗效果。方法 纳入2020年2月—2022年2月无锡虹桥医院消化内科收治的200例结肠息肉患者为研究对象,经随机数表法平均分组,每组100例。对照组接受高频电凝下金属夹内镜下结肠黏膜切除术,观察组给予氩等离子凝固术,对比两组临床疗效、血脂水平和生存质量。结果 观察组总有效率为96.00%,与对照组的91.00%比较,差异无统计学意义(χ^(2)=2.057,P=0.152);两组术前血脂水平和生存质量对比,差异无统计学意义(P>0.05)。复查时,观察组总胆固醇(5.12±0.72)mmol/L,三酰甘油(1.44±0.28)mmol/L,低密度脂蛋白胆固醇(4.12±1.24)mmol/L,均低于对照组,高密度脂蛋白胆固醇水平和生存质量总分高于对照组,差异有统计学意义(P<0.05)。结论 高频电凝下金属夹内镜下结肠黏膜切除术和氩等离子凝固术均可有效治疗结肠息肉,相比而言,后者在调节血脂水平,改善生存质量方面的作用更明显。Objective To compare the therapeutic effects of endoscopic colonic mucosal resection under high-frequency electrocoagulation with metal clips and argon plasma coagulation on colorectal polyps.Methods A total of 200 patients with colon polyps admitted to the Department of Gastroenterology,Wuxi Hongqiao Hospital from February 2020 to February 2022 were included in the study.They were divided into groups by random number table method,with 100 cases in each group.The control group received endoscopic colonic mucosal resection with metal clip under high frequency electrocoagulation,and the observation group received argon plasma coagulation.The clinical efficacy,blood lipid level and quality of life were compared between the two groups.Results There was no significant difference in the total effective rate between the observation group(96.00%)and the control group(91.00%)(χ^(2)=2.057,P=0.152).There was no significant difference in preoperative blood lipid level and quality of life between the two groups(P>0.05).At review,the total cholesterol,triglyceride and low density lipoprotein cholesterol in the observation group were(5.12±0.72)mmol/L,(1.44±0.28)mmol/L and(4.12±1.24)mmol/L,which were lower than those in the control group,and the level of high density lipoprotein cholesterol and the total score of quality of life were higher than those in the control group,and the difference was statistically significant(P<0.05).Conclusion Both endoscopic colonic mucosal resection with metal clips under high-frequency electrocoagulation and argon plasma coagulation can effectively treat colonic polyps.In comparison,the latter was more effective in regulating lipid levels and improving the quality of survival.
