最大强度投影CTA技术在急性缺血性卒中患者中的应用

摘  要:目的 通过比较时间最大强度投影CTA技术(t-MIP CTA)和单相CTA(sCTA)技术对急性缺血性脑卒中患者颅内血管影像质量的差异,探索t-MIP技术在急性缺血性卒中的应用.方法 回顾性收集在绍兴市人民医院接受一站式CT血管灌注扫描的急性缺血性卒中患者36例,应用sCTA和t-MIP CTA分别对灌注图像进行后处理,测量并计算两组后处理图像的衰减、信号噪声比、对比噪声比并进行主观图像质量评分.两组数据之间的比较运用配对样本t检验.结果 36例患者,共测量288条颅内前循环血管.两种后处理技术的图像均表现出良好的诊断质量.客观评价中,t-MIP CTA技术的血管衰减、信号噪声比(SNR)和对比噪声比(CNR)高于sCTA,差异有统计学意义(P<0.05).主观评价中,t-MIP CTA技术的分级:伪影、噪声、锐利度和诊断可接受高于sCTA,差异有统计学意义(P<0.05).结论 与sCTA技术相比,t-MIP CTA技术能更好显示颅内血管,提高影像质量,可以常规应用于急性缺血性卒中患者的血管评价.Objective To explore the preliminary application of t-mip technology in acute ischemic stroke by comparing the image quality of intracranial vessels in patients with acute ischemic stroke between time maximum intensity projection computed tomography angiography(t-MIP CTA)and single-phase CTA(sCTA).Methods 36 patients with acute ischemic stroke who underwent one-stop CT vascular perfusion scanning in Shaoxing people's hospital were collected retrospectively.SCTA and t-MIP CTA were used to post processthe perfusion images respectively.The attenuation,signal-to-noise ratio and noise contrast of the two groups of post-processing images were measured and calculated,and the subjective image quality score was performed.Paired sample t-test was used to compare the two groups of data.Results A total of 288 anterior intracranial circulation vessels were measured in 36 patients.The images of the two post-processing techniques showed good diagnostic quality.In objective evaluation,the vascular attenuation,signal noise and noise contrast of t-MIP CTA were significantly higher than those of sCTA,and the dfference between the two was statistically significant(P<0.05).In the subjective evaluation,the artifact,noise,sharpness and diagnostic acceptability of t-MIP CTA were significantly higher than those of sCTA,and the difference between the two was statistically significant(P<0.05).Conclusion Compared with sCTA,t-MIP CTA can better display intracranial vessels and improve image quality.It can be routinely used in vascular evaluation of patients with acute ischemic stroke.
