固定矫治对根管治疗牙牙根吸收的影响和相关因素分析

摘　　要：目的 探讨固定矫治对根管治疗牙牙根吸收的影响和相关因素。方法 分析2018年1月至2020年6月商丘市中医院在接受正畸固定矫治前实施口内根管治疗患者130例的临床资料,在为患者开展正畸治疗前后,先实施锥形束CT检查,对矫治后患牙的牙根吸收长度予以测量,分析固定矫治对根管治疗牙牙根吸收情况及影响因素。结果 根管治疗后患牙的前牙牙根吸收量为(0.93±0.30)mm,后牙牙根吸收量(0.60±0.18)mm,与对侧同名活髓牙牙根吸收量[(1.10±0.26)mm,(0.80±0.18)mm]比较差异有统计学意义(P<0.05);儿童、后牙的牙根吸收量少于成年人、前牙,差异有统计学意义(P<0.05);多因素logistic回归分析显示,年龄(r=3.605)、牙位(r=2.471)为影响根管治疗牙牙根吸收的危险因素(P<0.05)。结论 固定矫治后根管治疗牙的牙根吸收率低于对侧活髓牙,固定矫治后根管治疗牙牙根吸收主要受年龄及牙位影响。Objective To investigate the influence of fixed orthodontics on root absorption and related factors in root canal treatment.Methods Retrospective analysis the clinical date of 130 cases who undent endooral root canal treatment before fixed orthodontics in Shangqiu Traditional Chinese Medicine Hospital from January 2018 and June 2020, conduct orthodontic treatment on patients before and after the implementation of cone beam CT examination, first to treatments for tooth root absorption length measurement, analysis of the fixed appliance fang root absorption of root canal treatment situation and influencing factors.Results After root canal treatment, the absorption of anterior and posterior teeth was(0.93±0.30) mm and(0.60±0.18) mm, respectively, and the difference was statistically significant(P<0.05) compared with the same living pulp tooth on the opposite side [(1.10±0.26) mm,(0.80±0.18) mm].The amount of root absorption of children and posterior teeth was less than that of adults and anterior teeth, and the difference was statistically significant(P<0.05).Multivariate logistic regression analysis showed that age(R=3.605) and tooth position(r=2.471) were the risk factors affecting root resorption after root canal treatment(P<0.05).Conclusion The absorption rate of root canal treated teeth was lower than that of contralateral live pulp teeth.The absorption rate of root canal treated teeth was mainly affected by age and tooth position.
