摘  要:目的利用三维有限元分析的方法探究使用改良C型腭侧板(modified C-palatal plate,MCPP)远中移动上颌牙列时牙齿在不同载荷方向下的移动趋势及生物力学机制.方法建立使用MCPP远中移动上颌牙列的三维有限元模型,将MCPP牵引凹槽高度设置为距离牙槽嵴顶2 mm、5 mm、8 mm,牵引钩为距离牙槽嵴顶1 mm、4 mm、7 mm.通过有限元分析的方法,在不同高度牵引凹槽与牵引钩之间施加3 N的牵引力,并比较牙齿的移动情况.结果9种载荷方向下,上颌牙列均出现不同程度的顺时针旋转,第一磨牙近中颊尖较中切牙切缘压低了(0.012~0.030)mm.在不同载荷方向下,第一磨牙近中腭尖均向颊侧[(0.002~0.054)mm]和远中位移[(0.012~0.060)mm],并有牙弓增宽.在MCPP牵引凹槽距牙槽嵴顶8 mm,牵引钩高度为1mm时,中切牙切缘唇侧位移0.002 mm,其他载荷方向下中切牙切缘出现(0.005~0.050)mm的内收.结论使用MCPP远中移动上颌牙列时,在不同的载荷方向下,上颌牙列均会出现顺时针旋转,牙弓宽度增加.Objective To analyze the movement trend and the biomechanical mechanism of tooth in maxillary arch distalization through modified C-palatal plates(MCPP)with different loading position using three-dimensional finite element analysis.Methods Three-dimensional finite element model of maxillary arch distalization using modified C-palatal plates was established.The vertical position of MCPP loading notches were 2 mm,5 mm and 8 mm above the alveolar ridge and the heights of the palatal arch hook were 1 mm,4 mm and 7 mm.By using 3-dimensional finite element analysis,3N retraction forces were applied between the different notches and hooks to compare the tooth movement.ResultssThe maxillary dentition showed different degrees of clockwise rotation with all 9 loading directions and the mesiobuccal cusp of the first molars had more intrusion(0.012~0.030 mm)than the incisal edge of central incisors.Under different loading directions,the mesiopalatal cusp of first molars all had buccal[(0.002~0.054)mm]and distal[(0.012~0.060)mm] movement,with the width of the dental arch increased.When the MCPP notches were 8 mm above the alveolar ridge with the 1 mm high hooks,the incisal edge of central incisor moved 0.002 mm labially.The incisal edge of central incisors retracted(0.005~0.050)mm under other loading directions.Conclusions The maxillary dentition,with different loading directions,had a clockwise rotation and an increasement in the width of the dental arch when distalizing maxillary dentition with MCPP.
