两种拔牙创处理方式在后牙位点保存中应用的疗效评价

摘　　要：目的:评价在后牙位点保存中运用游离龈组织(free gingival graft,FGG)或浓缩生长因子(concentrated growth factor,CGF)膜处理拔牙创的疗效。方法:选取有单颗后牙拔除后拟行位点保存术的患者36例,随机分至FGG组和CGF组,每组18例。微创拔牙彻底清创后FGG组采用Bio-Oss骨粉+海奥生物膜+FGG;CGF组采用Bio-Oss骨粉+海奥生物膜+CGF膜。比较两组患者治疗效果及术后疼痛情况。结果:FGG组和CGF组术前与术后6个月在角化龈宽度、牙槽骨垂直高度及水平宽度变化之间差异无统计学意义(P>0.05);CGF组在术后3 d及术后1周的术后疼痛显著小于FGG组(P<0.05)。结论:运用FGG和CGF膜处理拔牙创可以取得相似的牙槽窝软、硬组织轮廓维持效果,且CGF膜的运用可以有效减轻患者术后疼痛,是良好的拔牙创处理材料。Objective:To evaluate the efficacy of free gingival graft(FGG)or concentrated growth factor(CGF)membrane in the ridge preservation of posterior teeth.Methods:Thirty-six patients with single posterior tooth requiring extraction who planned to undergo site preservation were randomly divided into FGG group and CGF group,with 18 cases in each group.After minimally invasive tooth extraction and complete debridement,the FGG group was treated with Bio-Oss+Haiao biofilm+FGG.The CGF group was treated with Bio-Oss+Haiao biofilm+CGF membrane.The treatment effect and postoperative pain were compared between the two groups.Results:There was no significant difference in the width of keratinized gingiva,vertical height and horizontal width of alveolar bone between FGG group and CGF group before and 6 months after operation(P>0.05).Postoperative pain at 3 days and 1 week after surgery in CGF group was significantly lower than that in FGG group(P<0.05).Conclusion:Using FGG or CGF membrane to treat tooth extraction wound can achieve similar soft and hard tissue contour maintenance effect of alveolar fossa,and the use of CGF membrane can effectively reduce postoperative pain of patients,which is a good material for tooth extraction wound treatment.
