胃食管反流病内镜下治疗术后的食管敏感性分析

摘　　要：目的分析胃食管反流病(GERD)内镜下治疗术后的食管敏感性。方法选取2020年11月至2021年11月就诊于河南科技大学第一附属医院消化内科,拟行内镜下治疗(射频消融、黏膜切除术、黏膜套扎术)的胃食管反流病患者20例,收集其入院时和术后6个月的血清标本、食管组织标本,调查术前术后各个时间段血清、食管组织标本中PAR-2、TRPV1的表达情况,统计各个时间点反流症状指数(RSI)、GERD-Q的评分。结果术后6个月RSI及GERD-Q评分较术前降低;术后食管组织的炎性细胞浸润较术前明显减少;术后PAR-2及TRPV-1免疫染色阳性着色细胞染色浅并且稀疏,术后食管黏膜中PAR-2和TRPV-1的蛋白水平较术前低;胃食管反流病患者术后血清PAR-2及TRPV-1的表达量降低。结论内镜下治疗胃食管反流病能有效缓解患者的临床症状,改善患者组织学损伤,降低食管的敏感性,且临床症状的缓解与食管敏感性受体的表达呈正相关。Objective To analyze the esophageal sensitivity after endoscopic treatment of gastroesophageal reflux disease(GERD).Methods 20 patients with GERD who were admitted to our hospital from November 2020 to November 2021 and received endoscopic treatment(radiofrequency ablation,mucosal resection,and mucosal ligation)were selected.Their serum and esophageal tissue samples were collected at admission and 6 months after surgery.The expressions of PAR-2 and TRPV-1 in serum and esophageal tissue specimens at each time point before and after surgery were investigated,and the scores of reflux symptom index(lRSI),and GERD-Q at each time point were counted.Results The scores of RSI and GERD-Q in 6 months after operation were lower than those before operation;the infiltration of inflammatory cells in esophageal tissue after operation was significantly reduced compared with that before operation;in postoperative group,the expressions of PAR-2 and TRPV-1 cells were decreased;the expression level of PAR-2 TRPV-1 protein in esophageal mucosa after operation was lower than that before operation;the serum PAR-2 and TRPV-1 decreased in GERD patients after operation.Conclusion Endoscopic treatment of GERD can effectively relieve the clinical symptoms of patients,improve the histological damage,and reduce the sensitivity of esophagus.The relief of clinical symptoms is positively correlated with the expression of esophageal sensitivity receptors.
