胃黏膜及幽门螺杆菌双重染色在早期胃癌普查中的应用意义

摘　　要：目的分析胃黏膜及幽门螺杆菌双重染色在早期胃癌普查中的应用价值。方法选取于都县人民医院2019年3月至2020年8月行胃癌筛查的60例疑似早期胃癌患者,全部患者均接受胃黏膜及幽门螺杆菌双重染色检查,并行病理组织活检、^(14)C尿素呼气试验检查;比较幽门螺杆菌双重染色检查与^(14)C尿素呼气试验检查结果,且将病理组织活检结果作为“金标准”,分析胃黏膜及幽门螺杆菌双重染色在早期胃癌普查中的应用价值。结果幽门螺杆菌双重染色检查与^(14)C尿素呼气试验检查结果有极好的一致性(Kappa=0.832);60例疑似早期胃癌患者经病理组织活检检出27例早期胃癌,占45%(27/60);依据病理组织活检检查“金标准”,胃黏膜及幽门螺杆菌双重染色诊断早期胃癌敏感度、特异度、准确度分别为88.89%(24/27)、87.88%(29/33)、88.33%(53/60),且与病理组织活检检查结果有极好的一致性(Kappa=0.765)。结论胃黏膜及幽门螺杆菌双重染色在早期胃癌普查中具有较高应用价值,考虑未来可作为早期胃癌筛查辅助手段。Objective To determine the clinical value of double staining of gastric mucosa and helico-bacter pylori(Hp)in the screening of early gastric cancer.Methods Sixty patients with suspected early gastric cancer,who underwent gastric cancer screening in Yudu County People′s hospital between March 2019 and August 2020,were included in the study.All patients received double staining of gastric mucosa and Hp.Pathological tissue biopsy and^(14)C urea breath test were performed.The results of Hp double staining and^(14)C urea breath test were com-pared,and the pathological biopsy results were used as the"gold standard"to analyze the clinical value of gastric mucosa and Hp double staining in the screening of early gastric cancer.Results The results of Hp double stain-ing and^(14)C urea breath test showed excellent consistency(Kappa=0.832).Among the 60 patients with suspected early gastric cancer,27 were detected by pathological tissue biopsy,accounting for 45%(27/60).According to the"gold standard"of pathological biopsy,the sensitivity,specificity and accuracy of gastric mucosa and Hp double staining in the diagnosis of early gastric cancer were 88.89%(24/27),87.88%(29/33)and 88.33%(53/60),respectively,and there was excellent consistency with the results of pathological biopsy(Kappa=0.765).Conclusion The double staining of gastric mucosa and Hp shows high clinical value in the screening of early gastric cancer,and it may be considered as an auxiliary method for screening early gastric cancer in the future.
