顺铂与氟尿嘧啶热化疗治疗老年口腔颌面部恶性肿瘤的临床效果

摘　　要：目的探讨顺铂与氟尿嘧啶热化疗治疗老年口腔颌面部恶性肿瘤的临床效果。方法选取2016年6月至2019年1月我科室收治的82例口腔颌面部恶性肿瘤患者为研究对象,将其随机分为对照组和观察组,各41例。对照组给予顺铂与氟尿嘧啶化疗治疗,观察组在对照组基础上给予热疗治疗。比较两组的治疗效果。结果观察组的近期治疗总有效率为58.84%,高于对照组的34.15%,差异具有统计学意义(P<0.05)。治疗后,两组的白细胞分化抗原3阳性(CD3+)、白细胞分化抗原4阳性(CD4^(+))、白细胞分化抗原4阳性/白细胞分化抗原8阳性(CD4^(+)/CD8^(+))均升高,且观察组高于对照组,差异具有统计学意义(P<0.05)。治疗后,两组的口腔颌面部功能量表评分均升高,数字疼痛分级量表评分(NRS)评分均降低,且观察组优于对照组,差异具有统计学意义(P<0.05)。两组的毒副反应总发生率比较,差异无统计学意义(P>0.05)。治疗后,两组的癌症治疗功能总体评价量表(FACT-G)及口腔健康影响程度量表(OHIP-14)评分均升高,且观察组高于对照组,差异具有统计学意义(P<0.05)。观察组的满意度为95.12%,高于对照组的75.61%,差异具有统计学意义(P<0.05)。两组的2年生存率比较,差异无统计学意义(P>0.05)。结论顺铂和氟尿嘧啶热化疗治疗老年口腔颌面部恶性肿瘤的疗效显著,其可提高患者的近期疗效、免疫功能、口腔颌面部功能、生活质量及满意度,降低疼痛程度,且毒副反应与常规化疗相近,值得临床推广与应用。Objective To explore the clinical effect of cis-platin and fluorouracil thermochemotherapy in the treatment of oral and maxillofacial malignant tumors in the elderly.Methods A total of 82 patients with oral and maxillofacial malignant tumors admitted in our department from June 2016 to January 2019 were selected and randomly divided into control group and observation group,with 41 cases in each group.The control group was treated with cis-platin and fluorouracil chemotherapy,and the observation group was treated with hyperthermia on the basis of the control group.The therapeutic effects of the two groups were compared.Results The total effective rate of short-term treatment in the observation group was 58.84%,which was higher than 34.15%in the control group,and the difference was statistically significant(P<0.05).After treatment,the levels of cluster of differentiation 3 positive(CD3+),cluster of differentiation 4 positive(CD4^(+))and cluster of differentiation 4 positive/cluster of differentiation 8 positive(CD4^(+)/CD8^(+))in the two groups increased,those in the observation group were higher than the control group,and the differences were statistically significant(P<0.05).After treatment,the score of oral and maxillofacial function scale in the two groups increased,the score of Numerical Rating Scale(NRS)decreased,those in the observation group were better than the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the total incidence of toxic and side effects between the two groups(P>0.05).After treatment,the scores of Functional Assessment of Cancer Therapy-General(FACT-G)and Oral Health Impact Profile(OHIP-14)in the two groups increased,those in the observation group were higher than the control group,and the differences were statistically significant(P<0.05).The satisfaction of the observation group was 95.12%,which was higher than 75.61%of the control group,and the difference was statistically significant(P<0.05).There was no significant differe
