摘　　要：目的:通过自体血制备富血小板凝胶,探讨其在长期服用抗凝药物的患者拔牙手术中的临床应用。方法:将2019年1月—2020年12月在宜昌市第一人民医院口腔科进行拔牙治疗的长期服用抗凝药物的患者随机分为研究组和对照组,再选取未服用抗凝药物的正常患者作为空白组,每组各40名,观察三组患者拔牙术后2 h和24h的止血情况,术后1周软组织愈合情况,以及术后4周拔牙窝内牙槽骨密度。结果:研究组在术后2 h的止血、术后1周软组织愈合以及术后4周拔牙窝内牙槽骨密度等方面均优于对照组和空白组(P<0.05);术后24 h的止血情况,研究组优于对照组(P<0.05),但与空白组无明显差异(P>0.05)。结论:自体血制备富血小板凝胶,减少了拔牙术后出血,加快了软、硬组织的恢复,在临床上有较高的应用价值。Objective:To evaluate the platelet-rich gel prepared from autologous blood applicated in tooth extraction surgery for patients with long-term use of oral anticoagulants.Methods:The patients with long-term use of oral anticoagulants who were undergoing tooth extraction in our hospital from January 2019 to December 2020 were randomly divided into study group and control group,and normal patients who underwent tooth extraction without anticoagulants were selected as the blank group,with 40 patients in each group.The hemostatic status of the three groups of patients at the end of 2 h and 24 h after tooth extraction,the soft tissue healing one week after the operation,and the alveolar bone density in the extraction socket four weeks after the operation were analyzed.Results:The study group’s hemostasis at 2 h after operation,soft tissue healing after one week,and alveolar bone density in the extraction socket after 4 weeks were better than those of the control group and the blank group(P<0.05).In the study group,the hemostasis at 24 h after operation was better than that in the control group(P<0.05),but it showed no significant difference from that in the blank group(P>0.05).Conclusion:The preparation of platelet rich gel from autologous blood reduces postoperative bleeding and speeds up the recovery of soft tissues and bones,which has high clinical application value.
