摘 要:目的 分析台州市非结核分枝杆菌(NTM)的菌种分布和流行状况,为NTM病防治提供实验室依据。方法 对台州市2018年—2021年收集的1 437株分枝杆菌,先通过抗酸染色和MPB64抗原胶体金试验进行初步鉴定,然后分别用ITS、rpoB和hsp65基因测序方法进行菌种鉴定。结果 1 437株分枝杆菌中236株最终鉴定为NTM,总体分离率为16.4%;2018年—2021年分离率分别为7.0%、15.2%、17.2%、29.6%,呈上升趋势(χ^(2)趋势=54.90,P<0.05)。236株NTM被鉴定为22个菌种,以鸟分枝杆菌复合群(MAC)为主(72.9%),其中胞内分枝杆菌占57.2%;其次为脓肿分枝杆菌(9.7%)和堪萨斯分枝杆菌(4.2%)。感染者男女比例为1.56:1,主要集中在≥60岁人群。结论 台州地区NTM总体分离率处于较高水平,建议加强本地区NTM病监测并及时对菌种进行鉴定。Objective To provide laboratory basis of prevention and treatment of non-tuberculous Mycobacteria (NTM)disease,the epidemic states and distribution characteristics were analyzed.Methods A total of 1 437 Mycobacterium strains collected in Taizhou City from 2018 to 2021 were preliminarily identified by acid-fast staining and colloidal gold immunochromatography technology for MPB64,and then conducted for identification by ITS,rpo B and hsp65 gene sequencing.Results A total of 236 strains were confirmed as NTM and the isolation rate was 16.4%;the isolation rates of NTM in 2018-2021 were7.0%,15.2%,17.2%and 29.6%,respectively,showing an increasing trend (χ^(2)tendency=54.90,P<0.05).Twenty-two species were identified and the most dominant species was M.aviumcomplex (MAC)(72.9%),of which M.intercelleulare accounted for 57.2%,followed by M.abscessus (9.7%) and M.kansasii (4.2%).The infection was mainly concentrated in people aged≥60 years,and the ratio of male to female was 1.56∶1.Conclusion The isolation rate of NTM in Taizhou City was at a high level and it was recommended to strengthen the monitoring of NTM disease and identify the species in time.
