摘  要:目的 检测皮质性白内障患者晶状体中一氧化氮合酶(NOS)、沉默信息调节因子1(SIRT1)mRNA水平,分析二者与氧化应激的相关性及诊断价值。方法 选取2020年5月—2022年4月德清县人民医院收治的59例皮质性白内障患者,其中成熟期皮质性白内障患者38例(成熟期组)、过熟期皮质性白内障患者21例(过熟期组);另选取同期因意外导致眼部受伤需要做晶状体摘除手术的正常晶状体携带者27例为对照组。采用荧光定量PCR法检测晶状体中NOS、SIRT1 mRNA水平;酶联免疫吸附法检测晶状体中氧化应激指标[超氧化物歧化酶(SOD)、丙二醛(MDA)]水平;Pearson法分析皮质性白内障患者晶状体中NOS、SIRT1 mRNA水平与氧化应激指标的相关性;logistic回归分析影响皮质性白内障患者过熟期发生的因素;受试者工作特征曲线分析晶状体中NOS、SIRT1 mRNA水平对皮质性白内障患者过熟期发生的诊断价值。结果 对照组、成熟期组、过熟期组晶状体中NOS mRNA、MDA水平依次升高,SIRT1 mRNA、SOD水平依次降低(P<0.05)。皮质性白内障患者晶状体中NOS mRNA水平与SOD、SIRT1 mRNA水平成负相关,与MDA水平成正相关(P<0.05);晶状体中SIRT1 mRNA水平与SOD水平成正相关,与MDA水平成负相关(P<0.05)。NOS mRNA、MDA是皮质性白内障患者发生过熟期的危险因素(P<0.05),SIRT1 mRNA、SOD是皮质性白内障患者发生过熟期的保护因素(P<0.05)。晶状体中NOS mRNA联合SIRT1 mRNA诊断皮质性白内障患者过熟期发生的曲线下面积为0.959,高于二者单独诊断(P<0.05)。结论 皮质性白内障患者晶状体中NOS mRNA成高表达,SIRT1 mRNA成低表达,二者均与氧化应激指标有关,且可作为诊断皮质性白内障患者过熟期发生的有效指标。Objective This paper aims to detect the levels of nitric oxide synthase(NOS) and silent information regulator 1(SIRT1) mRNA in the lens of patients with cortical cataract, analyze the correlation between them and oxidative stress and discuss their diagnostic value. Methods A total of 59 patients with cortical cataract admitted to Deqing County People’s Hospital from May 2020 to April 2022 were selected, including 38 patients with mature stage cortical cataract(mature stage group) and 21 patients with overmature stage cortical cataract(overmature stage group). 27 normal lens carriers who needed lens extraction due to accidental eye injury were selected as the control group. The NOS and SIRT1 mRNA levels in lens were detected by real-time fluorescence quantitative PCR, the oxidative stress index [superoxide dismutase(SOD) and malondialdehyde(MDA)] levels in lens were detected by enzyme linked immunosorbent assay;the correlation between NOS and SIRT1 mRNA levels and oxidative stress index in lens of patients with cortical cataract was analyzed by Pearson method;logistic regression analysis was used to analyze the factors influencing the over-maturity of cortical cataract patients;receiver operating characteristic curve was used to analyze the diagnostic value of NOS and SIRT1 mRNA levels in lens for over-maturity of cortical cataract patients. Results The levels of NOS mRNA and MDA in the lens of control group, mature stage group and overmature stage group were successively increased, while the levels of SIRT1 mRNA and SOD were successively decreased(P<0.05). NOS mRNA level in lens of patients with cortical cataract was negatively correlated with SOD and SIRT1 mRNA level, and positively correlated with MDA level(P<0.05);SIRT1 mRNA level in lens was positively correlated with SOD level and negativelycorrelated with MDA level(P<0.05). NOS mRNA and MDA were the risk factors for the occurrence of hyper-maturity in patients with cortical cataract(P<0.05), while SIRT1 mRNA and SOD were the protective factors for the occ
