化瘀复明汤结合激光手术对DR患者黄斑中心凹厚度及视野缺损的影响

摘　　要：目的探讨化瘀复明汤结合激光手术对糖尿病视网膜病变(DR)患者黄斑中心凹厚度(CMT)及视野缺损的影响。方法78例DR患者,按随机数字表法分为对照组及观察组,各39例。对照组予以激光手术治疗,观察组予以激光手术结合化瘀复明汤治疗。比较两组治疗前后的CMT、最佳矫正视力(BCVA)、视野缺损情况[视野指数矫正模式标准差(CPSD)、周围视野的光敏感度(MS)、中心视野光敏感度平均缺损(MD)]、血液流变学指标[血浆粘度(PV)、红细胞沉降率(ESR)、红细胞聚集指数(AE)],并发症发生情况。结果治疗后,观察组的CMT(341.41±29.75)μm小于对照组的(368.04±32.48)μm,BCVA(0.89±0.15)高于对照组的(0.74±0.12),差异具有统计学意义(P<0.05)。观察组的CPSD(2.05±0.34)dB低于对照组的(2.87±0.36)dB,MS(1135.25±102.58)dB、MD(-1.15±0.23)dB高于对照组的(987.86±72.35)、(-2.11±0.35)dB,差异具有统计学意义(P<0.05)。观察组的PV(1.52±0.18)mPa·s、ESR(16.33±1.53)mm/h、AE(2.01±0.22)低于对照组的(1.63±0.21)mPa·s、(19.42±1.61)mm/h、(2.25±0.28),差异具有统计学意义(P<0.05)。观察组并发症发生率5.13%低于对照组的20.51%,差异具有统计学意义(P<0.05)。结论化瘀复明汤结合激光手术可提高DR患者的治疗效果,加快CMT、BCVA复常,减轻视野缺损程度,纠正血液流变学异常,且并发症少。Objective To discuss the effect of Huayu Fuming Decoction combined with laser surgery on central macular thickness(CMT)and visual field defect in diabetes retinopathy(DR)patients.Methods A total of 78 patients with DR were divided into a control group and an observation group according to the random numerical table,with 39 cases in each group.The control group was treated with laser surgery,and the observation group was treated with laser surgery and Huayu Fuming Decoction.Both groups were compared in terms of CMT,best corrected visual acuity(BCVA),visual field defects[correct pattern standard deviation(CPSD),mean sensitivity(MS),mean defect(MD)],blood rheological indexes[plasma viscosity(PV),erythrocyte sedimentation rate(ESR),erythrocyte aggregation index(AE)]before and after treatment,occurrence of complications.Results After treatment,the CMT of(341.41±29.75)μm in the observation group was less than that of(368.04±32.48)μm in the control group,and the BCVA of(0.89±0.15)was higher than that of(0.74±0.12)in the control group.The differences were statistically significant(P<0.05).The CPSD of(2.05±0.34)dB in the observation group was lower than that of(2.87±0.36)dB in the control group;the MS of(1135.25±102.58)dB and MD of(-1.15±0.23)dB in the observation group were higher than those of(987.86±72.35)and(-2.11±0.35)dB in the control group;the differences were statistically significant(P<0.05).The PV of(1.52±0.18)mPa·s,ESR of(16.33±1.53)mm/h,and AE of(2.01±0.22)in the observation group were lower than those of(1.63±0.21)mPa·s,(19.42±1.61)mm/h,and(2.25±0.28)in the control group,and the differences were statistically significant(P<0.05).The incidence of complications in the observation group was 5.13%,which was lower than that of 20.51%in the control group,and the difference was statistically significant(P<0.05).Conclusion Huayu Fuming decoction combined with laser surgery can improve the therapeutic effect of DR,accelerate the normalization of CMT and BCVA,reduce the degree of visual field defect
