双切口白内障囊外摘除术联合人工晶状体植入术治疗低角膜内皮细胞数硬核白内障患者的效果

摘　　要：目的:观察双切口白内障囊外摘除术(ECCE)联合人工晶状体植入术治疗低角膜内皮细胞数硬核白内障患者的效果。方法:回顾性分析2019年2-12月76例低角膜内皮细胞数硬核白内障患者(76眼)的临床资料,按不同ECCE切口分为观察组和对照组各38例。对照组采用单切口ECCE联合人工晶状体植入术治疗,观察组采用双切口ECCE联合人工晶状体植入术治疗,比较两组患眼视力、散光度、角膜内皮细胞计数、六边形细胞比例、角膜内皮细胞丢失率和并发症发生率。结果:术前和术后6个月,两组视力、散光度比较,差异无统计学意义(P>0.05);术后6个月,观察组角膜内皮细胞计数和六边形细胞比例均高于对照组,角膜内皮细胞丢失率低于对照组,差异有统计学意义(P<0.05);观察组并发症发生率为10.53%,低于对照组的31.58%,差异有统计学意义(P<0.05)。结论:双切口ECCE联合人工晶状体植入术治疗低角膜内皮细胞数硬核白内障患者可提高术后角膜内皮细胞计数和六边形细胞比例,降低角膜内皮细胞丢失率和并发症发生率,效果优于单切口ECCE联合人工晶状体植入术治疗。Objective:To observe effects of double incision extracapsular cataract extraction(ECCE)combined with artificial lens implantation in treatment of hard nucleus cataract patients with low corneal endothelial cell counts.Methods:The clinical data of 76 hard nucleus cataract patients(76 eyes)with low corneal endothelial cell counts from February to December 2019 were retrospectively analyzed.According to different ECCE incisions,they were divided into observation group and control group,38 cases in each.The control group was treated with single incision ECCE combined with artificial lens implantation,while the observation group was treated with double incision ECCE combined with artificial lens implantation.The visual acuity of the affected eye,the astigmatism degree,the corneal endothelial cell count,the hexagonal cell ratio,the corneal endothelial cell loss rate,and the incidence of complications were compared between the two groups.Results:There were no significant differences in the visual acuity and the astigmatism degree between the two groups before and 6 months after the surgery(P>0.05).6 months after the surgery,the corneal endothelial cell count and the hexagonal cell ratio in the observation group were higher than those in the control group,the corneal endothelial cell loss rate was lower than that in the control group,and the differences were statistically significant(P<0.05).The incidence of complications in the observation group was 10.53%,lower than 31.58% in the control group,and the difference was statistically significant(P<0.05).Conclusions:Double incision ECCE combined with artificial lens implantation can improve the corneal endothelial cell count and the hexagonal cell ratio in the hard nucleus cataract patients with low corneal endothelial cell counts,and reduce the corneal endothelial cell loss rate and the incidence of complications.Moreover,it is superior to single incision ECCE combined with artificial lens implantation.
