摘 要:目的探究不同新型机用镍钛器械进行根管预备后对牙本质微裂形成的影响。方法选取2020年12月至2022年6月在厦门医学院附属口腔医院因正畸拔除的单根管离体下颌前磨牙80颗(第一前磨牙),按照随机数字表法分为对照组、PTU组、PTG组、WOG组,每组各20颗。对照组不进行根管预备,其他3组分别用ProTaper Universal(PTU组)、ProTaperGold(PTG组)、WaveOneGold(WOG组)3种不同机用镍钛进行根管预备。比较各种镍钛器械所致的微裂牙数差异及3、6、9 mm 3个横截面产生的微裂牙数之间的差异,对比各组根尖孔偏移率。结果对照组患牙均无裂,PTU组、PTG组、WOG组均存在不完全裂,四组患牙裂纹情况比较,差异有统计学意义(P<0.05),PTU组发生不完全裂牙数最多。PTG组、PTU组、WOG组3 mm截面不完全裂牙数比较,差异有统计学意义(P<0.05);三组的6、9 mm截面不完全裂牙数比较,差异无统计学意义(P>0.05)。PTG组、PTU组、WOG组根尖孔偏移率比较,差异有统计学意义(P<0.05),PTG组根尖孔偏移率最低。结论ProTaperUniversal、ProTaperGold、WaveOneGold 3种新型机用镍钛器械在进行根管预备时产生不同程度的牙本质微裂,且在距根尖3mm处微裂纹明显增加,ProTaperGold在降低根尖孔偏移率、减少牙本质微裂方面具有较大优势。Objective To investigate the effect of different nickel-titanium instruments on the formation of dentin microcracks in root canal preparation.Methods A total of 80 single-root canal mandibular premolars(first premolars)extracted for orthodontic reasons in the Stomatological Hospital of Xiamen Medical College from December 2020 to June 2022 were selected and divided into control group,PTU group,PTG group and WOG group according to the random number table method,with 20 in each group.ProTaperUniversal(PTU group),ProTaperGold(PTG group)and WaveOneGold(WOG group)were used to prepare the root canal.The differences in the number of microfractured teeth caused by various niti instruments,the differences in the number of microfractured teeth caused by 3 mm,6 mm,and 9 mm cross sections,and the deviation rate of apical foramen in each group were compared.Results There were no cracks in the control group,while incomplete cracks were found in the PTU,PTG,and WOG groups.The difference in tooth cracks among the four groups was statistically significant(P<0.05),and the number of incomplete cracks in the PTU group was the highest.There was significant difference in the number of incomplete cracked teeth at 3 mm section among PTG,PTU,and WOG groups(P<0.05).There were no significant differences in the number of incomplete cracked teeth at 6 mm and 9 mm sections among the three groups(P>0.05).The deviation rate of apical foramen among PTG,PTU,and WOG groups was statistically significant(P<0.05),and the deviation rate of apical foramen in PTG group was the lowest.Conclusion Different degrees of dentin microcracks were observed during root canal preparation by three new mechanical niti instruments,ProTaperUniversal,ProTaperGold and WaveOneGold,and the microcracks increased significantly at 3mm from the root tip.ProTaperGold has great advantages in reducing the rate of apical foramen deviation and reducing dentin microcracks.
