肝细胞癌微血管癌栓、门静脉癌栓形成与Ki-67表达水平相关性分析

摘　　要：目的探讨肝细胞癌微血管癌栓(MVI)及门静脉癌栓(PVTT)形成与Ki-67表达水平之间的相关性。方法回顾性分析100例经手术或穿刺证实的肝细胞癌患者的临床、病理及影像特征资料,所有病例分为四组MVI(-)组38例、MVI(+)组35例、PVTT(-)组8例及PVTT(+)组19例。组间比较采用Mann-Whitney U检验,检验进行单因素秩和检验分析,分析微血管癌栓及门静脉癌栓等级与Ki-67表达水平之间的相关性及相关程度。结果MVI(-)组,MVI(+)组、PVTT(-)组及PVTT(+)组其平均Ki-67表达水平分别为15.2%、35.3%、33.5%、29.2%。组间比较结果显示,MVI(-)组与MVI(+)组、MVI(-)组与PVTT(-)组及MVI(-)组与PVTT(+)组比较其Ki-67表达水平有统计学差异(P<0.05),其余组比较其Ki-67表达水平无统计学意义(P>0.05)。结论肝细胞癌微血管癌栓及门静脉癌栓形成与Ki-67表达水平相关,且随着Ki-67表达水平的升高微血管癌栓及门静脉癌栓形成的概率也越大,Ki-67表达水平可在一定程度上反映微血管癌栓及门静脉癌栓形成的等级。Objective To investigate the correlation between the formation of Microvascular Invasion(MVI)and portal vein tumor thrombus(PVTT)and ki-67 expression level in hepatocellular carcinoma.Methods The clinical,pathological and imaging characteristics of 100 patients confirmed by operation or puncture were retrospectively analyzed.All patients were divided into four groups(38 cases in MVI(-)group,35 cases in MVI(+)group,8 cases in PVTT(-)group and 19 cases in PVTT(+)group).Comparison among groups was performed by Mann-Whitney U test.Single-factor ranksum test analysis was performed to analyze the correlation between the grade of MVI,PVTT and Ki-67 expression level and the degree of correlation.Results The average expression level of Ki-67 in MVI(-)group,MVI(+)group,PVTT(-)group and PVTT(+)group was 15.2%,35.3%,33.5%and 29.2%,respectively.There were significant differences of Ki-67 expression among MVI(-)group and MVI(+)group,MVI(-)group and PVTT(-)group,and MVI(-)group and PVTT(+)group(P<0.05),and others were not significantly different of the Ki-67 expression(P>0.05).Conclusion The formation of MVI and PVTT in HCC is correlated with the expression level of Ki-67.With the increasing of Ki-67 expression level,the probability of MVI and PVTT formation increases.The expression level of Ki-67 can reflect the grade of MVI and PVTT to a certain extent.
