摘　　要：目的:探讨多发性骨髓瘤(MM)患者骨髓单个核细胞中过氧化物酶体增殖物激活受体-γ共激活因子-1α(PGC-1α)mRNA表达水平及临床意义。方法:收集2018年2月-2021年12月上饶市人民医院收治的初治65例MM患者(MM组)和30例营养性贫血患者(对照组)的骨髓标本,提取单个核细胞,利用逆转录聚合酶链式反应(RT-PCR)法检测各组研究对象骨髓单个核细胞中PGC-1αmRNA表达情况,并分析其与临床相关指标之间的相关性。结果:MM组患者骨髓单个核细胞PGC-1αmRNA相对表达量显著高于对照组(P<0.05);MM组β_(2)-微球蛋白(β_(2)-MG)、乳酸脱氢酶(LDH)、C反应蛋白(CRP)及血钙等临床相关指标水平均显著高于对照组(P<0.05),MM组血红蛋白(Hb)水平显著低于对照组(P<0.05);经Pearson相关性分析,发现PGC-1αmRNA相对表达量与β_(2)-MG、LDH、血钙和CRP水平呈正相关(P<0.05),与Hb呈负相关(P<0.05)。结论:PGC-1αmRNA在临床上的表达与β_(2)-MG、LDH、血钙和CRP呈正相关,与Hb呈负相关,并且在MM骨髓单个核细胞中高表达,因此,认为可将其作为临床监测MM病情的一项重要指标。Objective:To investigate the expression level and clinical significance of peroxisome proliferator-activated receptor-γco-activation factor-1α(PGC-1α)mRNA in bone marrow mononuclear cells of patients with multiple myeloma(MM).Method:Bone marrow samples were collected from 65 MM patients(MM group)and 30 nutritional anemia patients(control group)admitted to Shangrao People's Hospital from February 2018 to December 2021.Mononuclear cells were extracted from the specimens to detect the expression level of PGC-1αmRNA by real-time fluorescent quantitative polymerase chain reaction(RT-PCR)method,and its correlation with clinical related indicators was analyzed.Result:The relative expression level of PGC-1αmRNA in bone marrow mononuclear cells of the MM group was significantly higher than that in the control group(P<0.05).The levels ofβ_(2)-microglobulin(β_(2)-MG),lactate dehydrogenase(LDH),C-reactive protein(CRP)and blood calcium in the MM group were significantly higher than those in the control group(P<0.05),while the level of hemoglobin(Hb)was significantly lower than that in the control group(P<0.05).Pearson correlation analysis found that the relative expression level of PGC-1αmRNA was positively correlated withβ_(2)-MG,LDH,blood calcium and CRP levels(P<0.05),and negatively correlated with Hb(P<0.05).Conclusion:The expression of PGC-1αmRNA is positively correlated withβ_(2)-MG,LDH,serum calcium and CRP in clinical practice,and negatively correlated with Hb,and is highly expressed in MM bone marrow mononuclear cells.Therefore,it is considered that it can be used as an important index for clinical monitoring of MM.
