语言发育迟缓儿童发育情况及影响因素分析

摘　　要：目的:探讨影响儿童语言发育的影响因素。方法:采用整群抽样抽取2018年9月-2021年9月在北京儿童医院顺义妇儿医院发育行为儿科就诊的语言发育迟缓儿童239例作为观察组,选取同期门诊中语言发育正常儿童151例作为对照组,运用单因素和多因素分析,找出儿童语言发育迟缓的发育特征和影响因素。结果:男童语言发育迟缓比例明显高于女童(P<0.05);不同性别发育迟缓儿童格塞尔发育量表各能区DQ比较,差异均无统计学意义(P>0.05);不同年龄发育迟缓儿童格塞尔发育量表各能区DQ比较,差异均有统计学意义(P<0.05)。单因素分析显示,性别、早产、每日使用电子产品>2 h、每日户外活动<2 h及感觉统合重度失调是儿童语言发育迟缓的影响因素(P<0.05)。logistic回归分析显示男童、早产、每日使用电子产品>2 h和感觉统合重度失调是影响儿童语言发育的危险因素(P<0.05)。结论:儿童语言发育受生物和社会心理因素等多方面影响,语言发育迟缓发现的时间越晚,越影响儿童智能发育水平,因此,在日后工作中对儿童语言发育迟缓做到早发现、早诊断、早治疗和早康复的同时,更应注重家长的健康教育。Objective:To explore the influencing factors of children's language development.Method:A total of 239 children with delayed language development who visited the Department of Developmental Behavioral Pediatrics,Shunyi Women and Children's Hospital of Beijing Children's Hospital from September 2018 to September 2021 were selected by cluster sampling as the observation group,and 151 children with normal language development in the outpatient department during the same period were selected as the control group.Univariate and multivariate analysis were used to identify the developmental characteristics and influence factors of delayed language development in children.Result:The proportion of delayed language development in boys was significantly higher than that in girls(P<0.05).There were no significant differences in DQ between different genders children with delayed language development in each functional area of Gesell developmental schedule(P>0.05).There were significant differences in DQ between different age children with delayed language development in each functional area of Gesell developmental schedule(P<0.05).Univariate analysis showed that gender,premature delivery,daily use of electronic products>2 h,daily outdoor activities<2 h and severe sensory integration dysfunction were the influencing factors of children's with delayed language development(P<0.05).logistic regression analysis showed that boys,premature delivery,daily use of electronic products>2 h and severe sensory integration dysfunction were the risk factors affecting children's language development(P<0.05).Conclusion:Children's language development is influenced by biological and social psychological factors,and the later the delayed language development is discovered,the more it affects children's intellectual development level.Therefore,in the future work,we should pay more attention to parents'health education while early detection,early diagnosis,early treatment and early rehabilitation of children's with delayed language development.
