儿童睑板腺囊肿与蠕形螨感染间的相关性研究

摘　　要：目的 回顾性分析儿童睑板腺囊肿与蠕形螨感染之间的相关性。方法 回顾性病例对照研究,收集2021年1月至2022年6月就诊于济南明水眼科医院小儿眼科门诊并完成活体共聚焦激光显微镜(vivo confocal microscopy, IVCM)睫毛根部蠕形螨感染情况检查的患儿109例。观察组:睑板腺囊肿儿童35例,对照组:健康儿童74例。收集年龄、性别、是否蠕形螨感染等信息分析睑板腺囊肿发生与蠕形螨感染的相关性。结果 观察组睫毛蠕形螨的感染率为91.43%,对照组为31.08%,两组比例差异有统计学意义(χ^(2)=34.617,P<0.001)。OR值为23.652,总体相对危险度的95%CI为(6.565, 85.217)。睑板腺囊肿的发生与蠕形螨感染有较好的相关性(V=0.564,P<0.001)。结论 与正常儿童相比,睑板腺囊肿患儿睫毛根部蠕形螨感染的发病率高。蠕形螨感染是睑板腺囊肿发生的危险因素,在儿童睑板腺囊肿的诊治过程中,应该注意蠕形螨的检查和驱螨虫治疗。Objective To investigate the correlation between demodicosis and chalazia in pediatric patients by the use of in vivo confocal microscopy(IVCM).Methods Retrospective case series.Thirty-five pediatric patients with chalazia and 74 children with normal ocular surface were included from pediatric ophthalmology clinic of Jinan Mingshui eye hospital from January 2021 to June 2022.In vivo laser microscopy has been performed on all subjects to examine the infection of demodex at the root of eyelashes.The the age,gender and demodex infection was collected to analyze the correlation of chalazia and ocular demodex infestation.Results The infection rate of demodex was 91.43%in the observation group and 31.08%in another.There was significant difference between the two groups(x^(2)=34.617,P<0.001)。The OR value was 23.652,and the 95%CI of the overall relative risk was(6.565,85.217).There was a good correlation between the demodex and the occurrence of chalazia(V=0.564,P<0.001).Conclusion There is a high prevalence of demodicosis in pediatric patients with chalazia,suggesting that ocular demodicosis is a risk factor for chalazia.We should pay attention to the ocular demodicosis in pediatric patients with chalazia.
