摘　　要：溃疡性结肠炎(ulcerative colitis, UC)是一种肠道非特异性炎症性疾病。中医对本病的描述见于痢疾、肠澼、泄泻等病,认为该病病位在大肠,又与五脏密切相关。UC以脾肾亏虚为本,湿热浊毒内蕴为标。韩捷教授在对本病的研究中认为,本病的症状表现及病因病机正契合《内经》通因通用思想在临床中的应用。UC虽以泄泻、排黏液脓血便等通为主要表现,但究其根本,存在湿热浊毒内阻、饮食内积、肝郁气滞、络脉瘀阻等脏腑气血“失通”的病机。通因通用法即治疗以通为表现,而本质为不通的病症。后世医家李中梓总结的治泻九法中的疏利法,即合通因通用思想,丰富了治疗泄泻之理论方法。韩捷教授在李中梓疏利法以及通因通用思想的启发下,进一步扩大了疏利法的思想内涵,将其应用到UC的治疗中。疏,即疏散调畅;利,即攻逐通利。脏腑气血宜通不宜滞。在治疗上擅于根据疾病不同时期病机特点辨证运用疏利之法。疾病初发期及复发期多湿热、气滞、食积,治疗上宜利湿热、导食积、通气机;慢性持续期多瘀血阻滞,宜行血滞,祛血瘀。临床上,韩捷教授运用疏利法治疗UC,使气血流通、五脏调和、六腑通降,机体恢复“通”的状态,邪却正安,利于UC恢复。Ulcerative colitic(UC)is a nonspecific inflammatory disease that mainly affects the intestine.The description of this disease in traditional Chinese medicine can be found in dysentery, diarrhea, intestinal aggregation and other diseases.It is believed that UC is located in the large intestine and is closely related to the five Zang-organs.The root of UC isspleen and kidney deficiency and the internal accumulation of damp-heat toxin is the secondary symptom.In the study of this disease, Professor HAN Jie consideres that the symptoms, etiology and pathogenesis of UC are fit with the application of the thought of treating diarrhea with purgative method in Neijing.Although UC is mainly manifested in catharsis, excretion of mucus, pus, blood and stool, the underlying pathogenesis of UC is the internal resistance of damp-heat turbidity toxin, internal accumulation of diet, liver depression and Qi stagnation, and blood stasis in collateral.The method of treating diarrhea by purgative method is to treat the disease which is characterized by diarrheawhile the root is obstruction.The method of soothing and purging is one of the nine methods of treating diarrhea summarized by later physician LI Zhongzi and is fit with the thought of treating diarrhea with purgative method.LI Zhongzi enriched the theoretical method of treating diarrhea.Inspired by the method of soothingand purging and the thought of treating the diarrhea with purgative method, Professor HAN Jie further expanded the ideological connotation of the method of soothing and applied it to the treatment of UC.Soothing means to evacuate and adjust and purging means todredge.The Zang-Fu organs, Qi and blood need to be dredged and should not be stagnant.In the treatment of UC,Professor HAN Jie is good at using the method of soothing and purging according to the different pathogenesis characteristics of the disease in different periods.Damp-heat, Qi stagnation and dyspepsia are more common in the incipient and relapsing stages of UC.Professor HAN Jie used the methods of
