复方大承气汤对粘连性肠梗阻患者CRP、ESR、NEU的影响

摘　　要：目的探讨复方大承气汤对粘连性肠梗阻的疗效及对血清C反应蛋白(CRP)、血沉(ESR)、中性粒细胞百分比(NEU)的影响。方法选取2019年5月—2021年5月医院收治的80例粘连性肠梗阻患者,随机数字表法分为观察组和对照组,各40例,对照组给予生理盐水灌肠,观察组给予复方大承气汤灌肠,比较两组临床疗效、临床指标、临床症状积分和血清CRP、ESR、白细胞计数(WBC)、NEU水平以及不良反应。结果观察组临床总有效率90.00%(36/40)显著高于对照组72.50%(29/40)(P<0.05);观察组首次通气时间、首次进食时间、肠鸣音恢复时间、腹部症状缓解时间、首次排便时间、总住院时间、总住院费用均少于对照组(P<0.05);治疗后观察组腹部胀痛、口苦口干、恶心呕吐、排便排气各症状积分均低于对照组(P<0.05);治疗后观察组血清CRP、ESR、WBC、NEU水平均低于对照组(P<0.05);两组不良反应发生率比较差异无统计学意义(P>0.05)。结论复方大承气汤灌肠治疗粘连性肠梗阻可有效改善临床症状,缩短恢复时间,并可降低血清CRP、ESR、WBC、NEU水平,临床疗效显著且安全性好。Objective To explore the curative effect of Compound Dachengqi Decoction(复方大承气汤)on adhesive intesti⁃nal obstruction,and its influence on serum C-reactive protein(CRP),erythrocyte sedimentation rate(ESR)and neutrophil per⁃centage(NEU).Methods A total of 80 patients with adhesive intestinal obstruction admitted to the hospital were enrolled between May 2019 and May 2021.According to random number table method,they were divided into observation group and control group,40 cases in each group.The control group was given normal saline,while the observation group was given Compound Dachengqi Decoction for enema.The clinical curative effect,clinical indexes,scores of clinical symptoms,levels of serum CRP,ESR,white blood cell count(WBC)and NEU,and adverse reactions between the two groups were compared.Results The total clinical re⁃sponse rate of the observation group was significantly higher than that of the control group[90.00%(36/40)vs 72.50%(29/40)](P<0.05).The first ventilation time,the first eating time,recovery time of bowel sound,relief time of abdominal symp⁃toms,the first defecation time,total hospitalization time and total hospitalization cost in the observation group were all lower than those in the control group(P<0.05).After treatment,the scores of symptoms(abdominal distention,bitter and dry mouth,nausea and vomiting,defecation and exhaust)in the observation group were lower than those in the control group(P<0.05),and the levels of serum CRP,ESR,WBC and NEU were lower than those in the control group(P<0.05).There was no significant differ⁃ence in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Compound Dachengqi Decoction can effectively improve the clinical symptoms,shorten the recovery time and reduce the levels of serum CRP,ESR,WBC and NEU in patients with adhesive intestinal obstruction,with significant clinical curative effect and high safety.
