地黄饮子联合舌项针治疗中风后假性球麻痹吞咽功能障碍临床疗效观察

摘　　要：目的探讨地黄饮子联合舌项针对中风后假性球麻痹吞咽功能障碍患者的影响。方法选取医院收治中风的后假性球麻痹吞咽功能障碍患者60例,随机分为两组,对照组给予常规治疗,治疗组在此基础上加用地黄饮子联合舌项针治疗,两组患者治疗周期为4周。观察两组患者洼田氏饮水试验评分、SSA、NIHSS及Barthel指数评分,检测患者治疗前后血清前白蛋白水平,并比较两组临床疗效。结果两组患者治疗7、14、28d后洼田氏饮水试验积分均下降,且治疗组下降程度更加显著(P<0.05)。治疗后两组SSA、NIHSS评分降低,与对照组相比较,治疗组SSA、NIHSS评分降低程度更加显著(P<0.05),Barthel指数升高,且治疗组升高指数更为明显(P<0.05)。两组患者治疗后血清前白蛋白水平均升高,且与对照组比较,治疗组提升更加显著(P<0.05)。治疗组总有效率是90.00%,对照组是70.00%,治疗组临床疗效更加显著(P<0.05)。结论地黄饮子联合舌项针治疗可发挥其疏通局部经络气血的作用,又具有滋肾阴,补肾阳,开窍化痰之效,使气血阴阳平和,脉络通利,对中风后假性球麻痹吞咽功能障碍者临床疗效显著,值得临床推广应用。Objective To investigate the effect of Dihuang Yinzi(地黄饮子)combined with tongue-nape acupuncture on pa⁃tients with swallowing dysfunction after stroke with pseudobulbar paralysis.Methods Sixty patients with dysphagia after pseudobul⁃bar paralysis in the hospital were randomly divided into two groups.The control group was given conventional treatment,and the treatment group was treated with Dihuang Yinzi combined with tongue-nape acupuncture on the basis of this treatment.The treat⁃ment cycle of the two groups was 4 weeks.The scores of depression drinking water test,Standardized Swallowing Assessment(SSA),National Institute of Health stroke scale(NIHSS)and Barthel index were observed in two groups,and the serum pre-al⁃bumin level before and after treatment was detected,and the clinical efficacy of the two groups was compared.Results After 7,14 and 28 days of treatment,the scores of water test in both groups decreased,and the decrease degree of in the treatment group was more significant(P<0.05).After treatment,the scores of SSA and NIHSS in the two groups decreased,and compared with those of the control group,the scores of SSA and NIHSS in the treatment group decreased more significantly(P<0.05),while Barthel index increased,and the index increased more significantly in the treatment group(P<0.05).Serum prealbumin level in both groups increased after treatment,and compared with that in the control group,the improvement in the treatment group was more significant(P<0.05).The total effective rate was 90.00%in the treatment group and 70.00%in the control group,and the clin⁃ical effect was more significant in the treatment group(P<0.05).Conclusion The treatment of Dihuang Yinzi combined with tongue-nape acupuncture can play the role of dredging Qi and blood of local meridians,and has the effect of nourishing kidney Yin,tonifying kidney Yang,activating the orifice and resolving phlegm,making Yin and Yang of Qi and blood calm,dredging col⁃laterals.It has remarkable clinical effect on patients with swa
