温胆汤对改善慢粒并发精神分裂症患者化疗后不良反应及精神症状的疗效研究

摘　　要：目的 探讨温胆汤对改善慢性粒细胞性白血病(慢粒)并发精神分裂症患者化疗后不良反应及精神症状改良方面的效果。方法 回顾性分析了2018年7月—2021年9月的5例慢粒患者,在服药治疗期间被诊断为精神分裂症。患者在接受羟基脲治疗慢粒的过程中,服用利培酮控制精神症状,由于不良反应较大,采用温胆汤进行治疗,期间监测病人的血相变化,对患者不良反应利用量化表进行评分,以阳性与阴性症状量表(PANSS)评分作为患者精神状态的评估标准。结果 服用温胆汤后,患者由于服药产生的恶心、干呕、头痛等症状明显减轻,量化评分下降明显。在相同利培酮服用量的情况下,患者的精神症状有进一步改善,PANSS评分有继续下降的趋势,在利培酮药量减半的情况下,服用温胆汤也可以较好地维持患者的精神症状。结论 温胆汤联合羟基脲,对于慢粒并发精神分裂症患者由于服药产生的不良反应具有很好的疗效,可减少利培酮的用药量,提高患者的生活质量。Objective To explore the effect of Wendan Decoction(温胆汤)on improving the side effects and mental symptoms of chronic myelogenous leukemia(CML)patients with schizophrenia after chemotherapy.Methods Five CML patients from July 2018 to September 2021 were analyzed retrospectively.They were diagnosed as schizophrenia during medication treatment.During the treatment of CML with hydroxyurea,the patients took risperidone to control their mental symptoms.Due to the serious side effects,Wendan Decoction was used for treatment.During the treatment,the routine blood test changes of the patients were monitored,the side effects of the patients were scored with a quantitative table,and the positive and negative symptom scale(PANSS)score was used as the evaluation standard of the patients'mental state.Results After taking Wendan Decoction,the symptoms of nausea,retching and headache caused by taking Wendan Decoction were significantly reduced,and the quantitative score decreased significantly.Under the same dosage of risperidone,the patients'mental symptoms were further improved,and the PANSS score continued to decline.When the dosage of risperidone was halved,taking Wendan Decoction can also better maintain the patients'mental symptoms.Conclusion Wendan Decoction combined with hydroxyurea has a good effect on the side effects of taking medicine in patients with CML complicated with schizophrenia,reducing the dosage of risperidone and improving the quality of life of patients.
