耳穴压豆治疗氯氮平所致精神分裂症患者便秘的临床疗效观察

摘　　要：目的 探讨耳穴压豆对于服用氯氮平导致精神分裂症患者便秘的临床疗效。方法 将2020年1月至2021年7月因诊断精神分裂症入院,住院期间伴有便秘,且目前仅服用氯氮平抗精神病治疗的50例患者纳入研究。入组后,对照组25例患者给予聚乙二醇电解质散口服,而观察组25例患者在对照组治疗基础上给予耳穴压豆治疗,两组均治疗14 d,通过对比两组治疗疗效,不良反应如皮炎、药疹情况,治疗后1个月患者服用氯氮平抗精神病治疗的依从性等来评估疗效。结果 经治疗,观察组及对照组在排便困难、便质干结、腹胀这3个症状上均有改善,观察组改善程度优于对照组(P <0.05)。两组患者慢性便秘严重度评分量表(CCS)评分比较,治疗后观察组CCS评分低于对照组,P <0.05。两组患者服药依从性对比,观察组治疗1个月后服药依从性明显高于对照组(P <0.05)。两组患者均未见不良反应。结论 耳穴压豆可有效缓解使用氯氮平抗精神分裂症治疗患者的便秘症状,且简便易行,不良反应少,可以改善患者服药依从性。Objective To investigate the clinical effect of auricular point pressure on constipation in patients with schizophrenia caused by clozapine.Methods From January 2020 to July 2021,50 patients diagnosed with schizophrenia who were admitted with constipation during hospitalization and currently only taking clozapine for antipsychotic treatment were included in the study.After inclusion,25 patients in the control group were given polyethylene glycol electrolyte powder orally,while 25 patients in the observation group were given auricular point pressure bean treatment on the basis of the control group.Both groups were treated for 14 days.The therapeutic efficacy,adverse reactions such as dermatitis and drug eruption,and the compliance of patients taking clozapine for antipsychotic treatment one month after treatment were compared to evaluate the efficacy.Results After treatment,the three symptoms of difficulty defecation,dry stool quality and abdominal distension were improved in both the observation group and the control group.The improvement degree of the observation group was better than the control group(P<0.05).As for the comparison of the evaluation of chronic constipation severity scale(CCS)between the two groups,and the improvement in the observation group was better than that in the control group,P<0.05.As for the comparison of medication compliance between the two groups,the difference of medication compliance between the two groups after 1 month of treatment was statistically significant,and the observation group was superior to the control group(P<0.05).No adverse effects were seen in two groups.Conclusion Auricular point pressure bean can effectively relieve the constipation symptoms of patients with antischizophrenia treatment using clozapine,and it is simple and easy,with few side effects,can improve the medication compliance of patients.
