产前超声心动图诊断胎儿单心房的临床意义

摘　　要：目的探讨产前超声心动图诊断胎儿单心房(SA)的临床意义。方法回顾性分析产前超声心动图诊断为单心房的11例胎儿,并观察心房类型、腔静脉和肺静脉与心房的连接、房室连接、合并心内外畸形情况及妊娠结局。结果①单心房分为3种类型:心房正位6例,心房反位3例,心房不定位2例。②腔静脉和肺静脉与心房异常连接共3例。③与单心房连接的心室分为2种:单心室9例,双心室2例。④11例胎儿全部合并心内畸形,8例合并心外畸形。⑤妊娠结局:11例胎儿全部选择引产,经病理解剖证实与产前超声诊断一致。结论产前超声心动图检查能准确诊断胎儿单心房的具体类型、腔静脉和肺静脉与心房的连接、房室连接以及合并心内外畸形情况,因此能为产前咨询和产后术前评估提供重要的临床信息。Objective To investigate the clinical significance of prenatal echocardiography in the diagnosis of fetal single atrium(SA).Methods 11 fetuses diagnosed as single atrium by prenatal echocardiography were analyzed retrospectively,and the atrial type,the connection between vena cava and pulmonary vein and atrium,the connection between atrium and ventricle,the complications of intra and extra cardiac malformations,and the pregnancy outcome were observed.Results①The single atrium was divided into three types:6 cases with atrium AP,3 cases with atrium AP,and 2 cases without atrium AP.②There were 3 cases of abnormal connection between vena cava and pulmonary vein and atrium.③The ventricles connected with single atrium were divided into two types:single ventricle(9 cases)and double ventricle(2 cases).④11 fetuses were all complicated with intracardiac malformations and 8 fetuses were complicated with extracardiac malformations.⑤Pregnancy outcome:All 11 fetuses selected induction of labor,which was confirmed by pathological anatomy to be consistent with prenatal ultrasound diagnosis.Conclusion Prenatal echocardiography can accurately diagnose the specific type of fetal single atrium,the connection between vena cava and pulmonary vein and atrium,the connection between atrioventricular and the combined cardiac and extracardiac malformations,so it can provide important clinical information for prenatal consultation and postpartum preoperative evaluation.
